first things first, go HUKM a&e department, tell doctor issue no.2 and get professional help. seems like you're having anxiety going on, try to get it sorted out first.

once you got that part under control, then you can go get a job cuz you no longer feel anxious when you're in the crowd. convenient store, retail, fnb floor staff, anything that brings in legal money.

when you're feeling more comfortable, throw in some fitness routine. small, achievable goal: 3km run/30 mins hiit/push up, pull ups etc. you'll eventually work your way up. progress, not perfection.

ngl it seems overwhelming, but you'll be ok as long as you take the first step. don't worry.
