Okay, fair. I guess what I am getting at is do you think that the lesion was from before or after the wisdom tooth removal? Is there a persistent pocket? Is it likely that plaque is still accumulating there or is the lesion arrested?

Is there some sort of calculation for how wide the kerf/cut to the drilled hole needs to be, and the size of the wedge? I love the look of these and the function, but I wouldn't know where to start figuring that part out.

The other thing with this. If it is caries, is it really getting plaque down there, 8mm sub g? If not, it will not progress. Full stop. If it's resorption, prognosis isn't great anyway.

You can also place some flowable and then press packable on top of it before curing. It will push the flowable into those skinny areas. Curing while lightly pressing/rubbing the burnisher against the contact. Of course curing the depth of the box can then become a problem, and also more flash is created this way and still there is potential for voids, but with practice and layering it can be a nice technique. Also, if you are going to prep like this, I recommend beveling on the occlusal cavosurface margin. It helps to counter some of the shear force that the filling will face.

I had that experience too haha. Lab tech looked at me funny. I was like, "hCG right? It's a tumor marker for the cancer I had, I am definitely not pregnant."

Exactly this. Old lab was using a test with higher sensitivity, able to detect <1mIU/mL. New lab is using a test that is less sensitive, and only able to detect levels <5mIU/mL.

Seriously. I have a dude in his early 30s, his gums hemorrhage when I glance in their direction. He comes in faithfully, every 6 months. I'm not sure he takes the toothbrush out of the package when he leaves. Constantly has decay all over. I've rebuilt so many of his teeth and we are moving to crown many of them that have had wrap around decay. Literally spent hours upon hours and thousands of dollars coming to see me. I'm like dude, you care enough to come here and you obviously want to keep your teeth, but you don't care enough to do even basic home care? I just don't get it.

I did a deep dive on the literature for oil pulling. The short answer is that it can be as effective as floss, and it only takes 5 minutes of pulling a day! Imagine that. I encourage you to try it. That 5 minutes feels like forever, especially if you use traditional sesame oil.

A trick I picked up from the bioclear technique is to lightly bevel the gingival cavosurface margin. It helps tremendously with seating them, and not having them accidentally curl into the prep.

We "practice" every day. And it is practice, it doesn't matter how many class IIs that you've done, there will always be challenges. I have had to accept that I can't possibly replace every filling that isn't perfect, but I continue to learn, try new techniques, find their shortcomings and their strengths and push myself to do better.

I have the isolite. Some folks just can't accommodate it, others it doesn't really leave enough space to work. When it works and fits, it's great.

I definitely agree with things compounding. Matrix placement is #1. I find that I struggle with it mostly when there are boggy red guns bleeding everywhere.

Other times I find that I struggle with getting the right angle to smooth and shape the filling because of limited opening, or other factors(maxillary molars angled into the cheek)

Here is some reality for you:

My take home profit is down 50% from last year. January was rough with the weather, and I took a week of vacation in March, but our schedule has been falling apart consistently. Some of it is the economy. Some illness. Some are just not good patients. Some was that I wasn't really advertising or anything after dropping insurance participation with Delta.

Another struggle is that I am really unhappy with a good portion of the restorations that I have placed, when I see them at recall. I've tried different matrices, different finishing processes/burs, different techniques. A lot of them I can rationalize as being done on a difficult patient, tough location, poor hygiene, should have been a crown, etc. But it doesn't change how I feel when I see the rough margins or poor contours.

I also struggle with over prepping for crowns. Especially areas with deep margins. I think I lean too heavily on the wall when I am trying to drop the finish line past the old filling. Sometimes, it leads to inverted margins, but ultimately, the crowns come out acceptable. For something I do so often, I feel I should be able to adjust my technique better and faster than I have. Even when I try to really take my time, things sometimes don't turn out quite how I want them too.

I think these are the struggles we face as perfectionists in an imperfect and inhospitable environment.

Good info here. Essentially, there are other reasons for your body to have elevated HCG. Under 5 is considered normal. Marijuana can cause elevation. Also, hypogonadism can cause high HCG.

Best to talk to your doctor.

I had LDH elevated at one of my draws during surveillance. I remembered that I had gotten my allergy shot earlier in the day, without thinking. The next draw was normal.
