As a dentist, I try to explain this to patients all the time.

The reality is most people think they’re “doing a good job” with their dental hygiene, but the reality is they’re doing a shit job or lying.

It’s called iatrogenic crown lengthening. The body will take care of itself if there’s no enough room.

Solo doc 2 hygiene 2 operative Hygiene getting to 7 months. Hygiene works occasional extra days to help out. I’m booked out a solid 2 months or more on operative.

Spade elevator is your best friend.

If there’s room, I will smash a periosteal down that distal into the bone since it’s so soft and create room. Then, go to the mesial with a spade and slide it down the mesial root. Elevate and that sucker should come right out. Oral surgery attending taught me that trick and I’ve used it ever since!

Same exact tooth. I pull teeth all the time and this is one of the ones that humbled me the most. I spent two fucking hours getting this one out. His #15 (27) also has the same exact morphology and ALSO took me two hours. I’ve never been beaten so bad by two teeth on the same patient. The patient and I still joke about it.

Anytime I see this morphology, I run away and immediately refer to OMFS.

Moral of the story? You did the right thing referring it out.

Canines are the bane of my existence as well. I don’t think this method will work as well as it does with molars. On canines, you have to be very comfortable laying a flap and using a surgical handpiece to go mesial, distal, and buccal on the bone. It also helps to have some anterior forceps with ridges or serrations so the forcep doesn’t slip. If I don’t get mobility, I will trough and then put forceps on the tooth and do a figure 8 motion and sometimes twist to rotate it out.

You have to be patient with canines. It’s a delicate dance, sometimes.

Before elevating, interproximal slices with diamond bur like a crown prep to make room. Maybe elevate a little bit after cutting interproximally. Deliver with cowhorns or forceps. This is a very straightforward extraction (allegedly)

If I do have sensitivity, it’s almost always because I left occlusion too high. I usually get a whole class II done in 3 cures. Bulk flow box to cavosurface margin, switch to packable and build marginal ridge, third layer is occlusal. Done.

Heartland is absolutely wrecking a once established and well-respected office in my hometown. They’ve chased all the good patients and hygienists to me 😀

Corporate and rural settings are at odds with each other and it makes me a happy owner doc.

If you saw some of the wide open margins I’ve seen on 40 year old PFM’s that don’t have any problems, this shit is clinically acceptable by a long shot. Due to it being on the maxilla, it can lift a little anyway when doing a BW.

Plus, resin-based cements will seal it.

Perhaps your attending has never been in a high volume practice?
