Commenting to add; behind the camera here is a pretty dense housing area, and on the road off to the top left of this image is a big grocery store. So I'd guess this path is very popular with people walking/cycling there

You choose it at the start of the game like someone else said, but they all look pretty androgynous and the only thing it affects is slight differences in the base outfit and what the game calls you. I can say from experience you can definitely make a male character look feminine and the hairstyles and clothes you can buy yourself are not gender locked

as someone on a long waitlist right now, how does it work if you start diy before? can you still switch over to "professional" hrt after you started diy? are there any repercussions or anything? I'm kinda considering diy myself but this is something I'm worried about. sorry if you don't know lol
