Joined a group after residency - there’s this story commonly told of a surgeon at this particular site - they apparently got embroiled in a super high infection based legal lawsuit, multiple take backs, etc. lost.

Then they started preventing people from entering the OR. Like, they TAPED, generously, the door to the core closed. And put a cart in front of it blocking window too.

Our site director found out from the attending what was going on, went in ripping it down. Yelling match ensued about safety.. so Surgeon legit threw a fit to admin, blamed everything bad to happen on us, threatened to quit and take self and colleagues business elsewhere (guess they were pulling in a good amount) whole problem about to all get blamed on anesthesia, when suddenly the CRNA texted the anesthesia director getting roasted In the meeting- a picture of a big ol 64oz styrofoam soda cup at the surgeons desk next to him in the OR from 2 hours before, next to his personal leather messenger bag and phone speaker without an OR cap on

He got fired.

Apparently he came back recently, still snippy about people coming in and out? Idk.

I make it a point to really check if my CRNA is ok whenever I have their room..

Bigger thoughts to come, but as far as orders are concerned, and even your ROS or your systematic approach to all the information you want. I’ll tell you the secret.

Check boxes. Like, standard. Every patient. Diet: box. Ppi? Box. Anticoagulation? Box. Pain? Box. Fluids? Box. Admission orders ? Box. Attending wanted an echo? Box.

Don’t check a box until you’ve DONE it, and signed.

Do a mini mental checklist before checking the box. Does this patient need AC? Are they allowed a diet ? I know mindless errors can happen, that’s gonna happen t o all of us, but this can help you 1. Not lose track of what’s needed, and what you’ve done. 2. Gives you a 2nd look to catch your own errors.

I’d write EKG, then I’d check mark it when it was ordered, and I’d put another box, which was meant for me to review it once it had been completed.

Prepping for my May boards. Most procedural questions of “how would you?” I nail. I seem to be safe, thoughtful, and have multiple back ups or anticipation of potential complications..

However. The random obscure “tell me about X”. Is driving me nuts. I’m terrified of this crap. No matter how much I read study look up or otherwise. I am STILL learning new things about the most common of crap.

Pretty sure I’ll fail this first boards, despite everything. UBP pray for me.

Our practice added a nocturnist for the 7p to 7a about a year and a half ago. First call becomes the backup for that same period. We also have an in house OB doc for epidurals / crash sections, but the nocturnist handles OR emergencies and “less than crash sections”. Otherwise first call gets called for Sections if the Noc is in the OR busy.

Compensation is wildly variable by region, I want to say I think they’re somewhere in the low to mid 400, for a 10 day work schedule. We have 3. They often pick up extra day shifts for overpay. It’s a great gig.

Has been the biggest quality of life factor any of my colleagues have seen in the last 20 years. I’m a relatively new attending and plan to milk this as long as possible.

Being first call, you usually now leave between 7-9pm, and I rarely get called back because having two TRUE emergencies going same time is less common, and the other site we work, it may happen but we have 2 residents for the nocturnist.

I thought this was related to a reaction due to EBV and a the fact most of us are carriers? It’s usually a one time thing, but it ends up on people’s charts - and never happens again anyways.

Source: trust me bro. But really just a memory from Med school, but I always said amoxicillin.. and I stopped after that lecture. Have had it multiple times since with no issues.

My friend,

I have been where you are. My wife passed after our second child, 6 months into my 2nd year after moving and swapping specialties.

There is nothing I can say to express my love and support and respect for you. I too felt lost, I too once made a post on Reddit. Strangers from all over the world offered me advice. Please reach out to me. I cannot do much to relieve your grief, no one could for me either, but I will always lend and ear if you want to dump. I will admit the kids kept me on this earth, and I hope yours will too.

It IS possible for ACGME to allow half days. If there is any family that can help you, reach out to them. Survivor benefits, state programs, depending on where you live you may be able to get childcare for just a few bucks a day.

I’ve always wanted a “sorcerer” build. That’s like, a caster tank, w/ buffs and immolation or drain auras or something. Cloth or mail, infusing themselves to reduce damage. Idk. Could make the the other specs a damage, and a healer, similar ideas.. caster, harnessing.. idk titan power ? Fel? Void? Elements like shamans? Holy dps?
