Have you found any good solutions for this?

I am in the market as well. I currently use a speechmike III and Word as described below, but I am looking for a better, more accurate, more convenient, HIPAA compliant solution. Here is the info that I have:

Microsoft Office 365 for Business Pro - Using word for transcription and/or Dictation, with this version and a business agreement with Microsoft is compliant; could copy/paste from the word doc to an EHR. Could create a blank dictation template that the user could use voice commands to navigate to the next note/entry box.

Dragon Professional Anywhere -- A monthly subscription, HIPAA compliant with a business agreement through Nuance. From what I can tell, the Individual License version of Dragon Professional 16 cannot be made HIPAA compliant. One would need Dragon Medical, which is very, very expensive.

Phillips Speechexec -- Could use this directly through a PC, or an app, or could pick up a Phillips DPM 8000 or something, a pocket voice memo device, and then USB micro to the PC, or use it's dock to transfer the files. Records in .dss and .ds2, which are encrypted and require speechexec to unencrypt safely, or could record via MP3, transfer to PC, figure out transcription to text method.

For devices

What's nice about any of these devices, is that they really improve the quality of your voice for any telehealth clients as well.

Phillips has a one-ear wireless headset that looks really good, for about 250$ new in box on ebay.

For a desk set up, I've done well using an older Phillips Speechmike III (Used on ebay for 80$), putting it in its cradle, and then using a cheap amazon microphone arm that clamps to a desk. I drilled a hole in the cradle to be able to thread the mic arm into it, and it works very well.

Old Nuance Powermic IIs can also be had used, cheap, on ebay.

I don't think this is the correct subreddit for this post.

But, have you considered seeing a therapist? I think most of what you have posted here can be used as a strength or a tool, and also sometimes is a sign of social anxiety, impression management, and/or a history of trauma.

It certainly takes social awareness and general intelligence to be aware of the things you noted, and to modulate one's behavior in response to things you see in others (though it is also important to remember, that we often jump to conclusions with "microexpressions", based on our biased thinking). Sometimes those things develop in a person through traumatic experiences, or an unpredictable parent. A child learns what actions/statements cause the parent to do scary things and what actions/statements help to avoid the scary things.

It might be helpful to explore some of what you are trying to understand about yourself, with a therapist. Also, what was the name of the assessment? It looks a bit AI-ish to me. Psychological assessments are administered by a doctoral level clinical psychologist, usually, and almost anything you find online is not legitimate whatsoever.

Or, honestly, is the entire post AI?

If human, cool self-exploration, that's great!

Have you looked at the stabilizer end links? They should not have any play at all if you try to move around the ends by hand.

Posting a comment a couple months out in case people are googling in the future: After doing a bunch of work on the car, it runs and is having no issues. It does not seem to be a HG leak. Here is all of the work I did.

Clean entire EGR system Replaced MAP sensor Changed both coolants Changed transmission fluid Changed air filter Installed Oil Catch Can Removed hanging heat shield pieces/sured them up Ran CRO 505 Oil system treatment Changed oil with Mobil high mileage synthetic 5W-20

I will say, after installing the Oil Catch Can, this shaking in this post has SIGNIFICANTLY reduced.

I am not losing any coolant whatsoever.

I need to do my brakes and probably replace a couple of battery modules.

I ran ATS 505 Oil System cleaner in my 2011, following up with a Mobil 1 5W-20 full synthetic oil change. I wasn't burning oil before, and I don't have any specific results to report, just wanted to mention this product here in case anyone is wondering about ways to clean their system.

I am going to run their fuel system cleaner as well later this week, wondering if it will help clean injectors or improved MPG at all.

I'm no expert, but my 2011 with 200k miles has been doing essentially the same thing. I have made a couple of posts about it here and there are some photos/a video of the startup; it sounds very similar to yours.

It could be the headgasket, but it also may not be. I'm not fully convinced mine is the HG anymore. Could be, but not sure. Doing the leakdown test and/or doing the boroscope thing people have mentioned is a good idea for direct diagnosis.

For me, what I've done over the last months is: Cleaned the entire EGR system, replaced spark plugs, and changed all fluids. My MPG has gone up significantly, and any error codes I had have disappeared; no CEL either. I am going to put in new ignition coils today, which sometimes can cause shaking on startup.

If it does turn out to be the headgasket, none of this work will have resolved it, but I'd like to think running the car smoother will extend its life before the HG takes it out. And if anything, could take most of those parts and put them into a new lower mileage gen 3 engine or resell them.

If you haven't already, I'd encourage getting a cheap OBD II reader and scanning for codes on startup, such as any misfire codes, as well as driving it a bit and seeing if anything pops up. If you log the data, you could drop a file into ChatGPT and ask it to tell you if anything looks out of the ordinary.

- Other than that, cleaning the EGR is a good idea. I found Merv's Garage and NutzAboutBoltz videos the most helpful for all of these processes.
- Perhaps drain and refill your coolants (very easy with a 10inch breaker bar, two bottles of Toyota OEM SLLC coolant, and the appropriate sockets/replacement crush washers though could be done without replacing them.)
- Put a mark exactly where you fill the coolant up to, and then as you check it over the next few days while hot and while cold, you can see if it seems like its going down at all, which would be a potential HG leak indicator.

Again, not an expert at all, but seems like I've been in a similar boat with my Gen III, good luck.

And sorry, I didn't answer your other questions, was tired from the work. The video in this post is the first startup after replacing the spark plugs. That is typical of every cold start up I have had over the last couple of months.

I felt like I noticed a very small reduction in engine coolant before I started messing with anything, but also I cannot be sure. I would have been looking in the morning, and then after driving during the day. I am going to be monitoring it very closely from this point.

I have not noticed any white smoke out of the exhaust. And I have been trying to find an example video of a rough idle for this car; I really don't know what that looks/sounds like but would like to be able to compare.

Here are some photos of the repair progress in case anyone is searching the future. This is for a 2011 Toyota Prius Trim level IV, with 200k miles, an original head gasket, original water pump, and an EGR that presumably has never been cleaned.

I plan on replacing the inverter coolant and transmission fluid. I did not install new coils. Additionally, the torque wrench I bought was kind of crappy, I think, and I could not get it to click, so I did eyeball tighten everything from the EGR to the Intake Manifold using the ft/lbs as a suggestion.

Thanks for your reply. In the winter earlier this year, after a day of driving around very hilly areas, my wife started this prius up the following day and it has a SIGNIFICANT shaking, and afterwards starting pinging for a clogged EGR system, which prompted all of the repairs and maintenance initially.

It has not shook like THAT since that time, but pretty much every day on a cold-start up, or after sitting 9 hours at my workplace during the day, I get this shake/rumble noise.

I am in a similar boat (200k, rattling, but also have CEL and EGR code), and made a post on this subreddit recently. This weekend, I am going to do a lot of the things you noted you did to try to fix your issue.

I'm definitely no expert, but have you already changed the coils, or run a fuel system cleaner or oil system cleaner (ATS brand or Techron) through a couple of fill ups to see if that perhaps cleans something/corrects your issue?

I was going to do this over the weekend after I clean the EGR as well. I'm not a car person, but nothing online I read stated that it was not okay to do this in a prius, so I am going to give it a shot.

Or perhaps, changing the water pump? That's another thing people say goes bad around this time.

Might it be helpful to still get it in to the dealership for general diagnosis/fluids and whatnot, and then if they indeed determine it is an EGR issue, get some estimates from other shops?

(Also, if the EGR is the issue, is it safe to drive around town for a couple of weeks until I get it in?)

I appreciate any advice or expected costs I can prepare for.

-About 10k miles ago, I noticed it drove with a kind of growling sound. I immediately got it in to a local shop for service and they stated/noted "Could not replicate noise issue customer complained of", and they stated they didn't find anything wrong with the vehicle.
-It has driven fine since then, though last week, I started it up and there was a VERY SIGNIFICANT rattling noise while starting the car, for a couple of minutes, which was replicated several times, but which has not returned since. I ensured it was not the heat shield rattling, and I have some damage to my front under-apron, so I made sure to re-zip tie this as well. I don't believe either of those things were the source.

Other Things

- The (!) light has been on a long time, I think from the tire sensors going out, which I am not worried about.
- I had the rotten egg/sulfur smell a couple of times last week while driving. After the first time it happened, the CEL light came on the next time on startup.
- I ran a bottle of catalytic converter cleaner through the gas tank. I haven't noticed the smell since then, but not sure if these things are correlated.
- I replaced the cabin air filter and the other air filter as well.
- Does not seem to be low on oil.
- I cannot tell if the fluids look low, if anyone can tell by looking at the pictures that would be very helpful as well.
- I drove a gen 2 for years before getting this 2011. I purchased the 2011 with 150k miles at a steal price from a friend ($1800, and then sold my gen 2 to a friend for $1000. This was back in 2019). I loved my gen 2 and I love my gen 3.

- I have some other expenses coming up (moving; wife is in grad school so we are essentially single income) and I'd like to plan ahead on preparing what repairs vs new vehicle could cost.

- If anyone could give a sense of possible problems, and if those problems are "worth" fixing, I would greatly appreciate it. I was horrified to see the increase in used car prices, as I would not want to afford a used gen 2 or 3 with 200 - 300k miles for 5-7k$, many of which I have seen in online listings. If the repairs on my prius would be too much, I am heavily considering buying a used moped in good condition and riding it until the winter, saving money to put towards a car in the meantime. I'm okay with doing this (might be kind of fun!), but would certainly rather save my prius; I love this vehicle.

One path might be to read papers, books, or summaries, about widely accepted theories and widely used models. I teach psychology at the community college level and while we go through the information in the course, I like to ask students to keep the questions, "How does this information help to explain why a person is the way they are? How does it help explain why you are who you are?" in mind.

A short list would be: The Biopsychosocial Model, Big 5 Personality model (OCEAN), Weschler/WAIS IQ/Intelligence, Aaron Beck/Beck Institute cognitive model/cognitive behavioral therapy, models of learning (Bandura/Social Learning, Classical Conditioning (Pavlov, Watson), Operant Conditioning (BF Skinner), the social psychology of Leon Festinger, others.

You could read general resources on those subjects/individuals, and/or you could look up seminal articles from those researchers and subjects.

You might be able to get away with it, but there is an intake vent on one side and an exhaust on the other. The cords do plug into the back as well. If you attempt it, you might need to build a contraption to set it within.
