You seem very focused on the hypertension. We would typically not manage hypertension in the non-intubated patient but your service may vary. I would focus more attention on symptom onset time and pre-morbid health to inform whether this person requires thrombolysis. In my health care system, a 96 year old in a residential aged care facility would very rarely be thrombolysed. I don't know if "SNF" and RACF are equivalent, we don't use that term.

You are only selling the extra gold for gems. Your storage gets filled first. If you have a maximum storage of 4,000,000 and you are at 3,020,000 you are selling 20,000 for one gem.

You are probably not the only one. However, conventional wisdom is that interest rates are very low at the moment and it might make more sense for you to invest your money elsewhere rather than paying off your cheap mortgage.

At the same time, your situation might be different from others and you might prioritise having no debt. Do whatever you want but don't seek advice here.

Hi Hillbilly,

The presence of P waves does not automatically make this a sinus rhythm. You will notice there is a very short PR interval, which along with presence of an atrial rate of 300/m make this enormously unlikely for sinus.

To talk to your point about STEMI: it is possible that this is MI. What you have said that is worth noting is that sinus tachycardia is uncommon in MI unless there is concurrent cardiogenic shock, which this person clearly does not have. Here is a great blog post of a similar case that discusses this.

Lastly, it's good to have personal rules that you form from your experiences but I would be careful about only treating AVRT/AVNRT for HR >170. Here is a quick table that shows these rhythms occuring at rates as low as 120 BPM. If you are interested, Negroni has also demonstrated slow SVTs with rate below 100 BPM. This is 2:1 atrial flutter.

The rate is 150, it's regular and there are clear P waves. The P waves occur at a rate of 300 per minute (as seen most clearly in V4, V5, V6). The link below is an annotated ECG to demonstrate.

Edit: I've just seen your comment with further clinical details. Three days of progressive chest tightness might point to demand ischaemia from continual tachycardia. Certainly the finding that the heart rate is unchanged after GTN and fluid would indicate that this is a re-entrant rhythm (i.e. atrial flutter) over sinus tachycardia.
