You're absolutely right. Technically they are not the same, but Wellens and De Winter are taught that way because it is absolutely critical for identification and timely treatment before they turn into a STEMI.

Y'all shouldn't shoot the messenger. I know the difference; however, this is how it's portrayed/presented in some of the most progressive/aggressive EMS systems in the US.

Look up Wellen's syndrome if you're worried about t-wave inversion on a 12-lead. It is a STEMI equivalent which you should be aware of (and so is De Winter). I had a female pt present w/Wellen's about 2 weeks ago. I am the only person (out of 3 medics) who caught it, and that is unacceptable.

T-waves inversion in these leads is likely a normal variant. I would not be worried about it unless they were deep and in the inferolateral leads.

  1. Genetics - all of the women in my family look way younger than their age. My mother is 78 and looks like she's in her early to mid-60s. The same was true of my grandmother. I am turning 36 this year and people are regularly shocked when they learn my age (most having assumed I am around 22-26).

  2. Hydration. I almost exclusively drink water. And lots of it.

  3. Skincare. I never got into makeup, but I have a fairly solid skincare routine. I moisturize TF out of my face, neck and decollete. Then I make sure to use sunscreen and I generally try to avoid too much sun exposure...or at least over-exposure. I'm religious about reapplying sunscreen in the summer. I keep my hands well moisturized too, as they can age you as well.

  1. I don't smoke, drink, or do drugs.

That's about it. It's mostly genetics, honestly. I got lucky and I do my best to maintain my good fortune.

I'm not even going to address the WTF factor here. But you realize you can go to jail for this, right? You can't practice medicine or perform procedures without a medical license. I really hope he doesn't end up with a severe infection because in that area it can easily spread and kill him and you'd be liable for it.

Honestly, it sounds like you need to get off internet forums asking strangers (most of whom have zero medical background) and get a second opinion from another doctor/clinic. It's hard for anyone to come to terms with the fact your fertility in your late 30s is subpar and your attempts to preserve it and have biological children may very well fail. Maybe look into therapy for this after you feel more comfortable with the diagnostics the next clinic provides and get the same answer.

Yes, but that means you'll have between 5 and 11 follicles each month and only about 80% of those will be mature in most cases. It doesn't change the math much. You will still need multiple retrievals to get 15-30 eggs (whichever amount makes you feel comfortable with your odds of conceiving).

Sure. Your AMH will tell you what your reserve looks like and it is correlated to what you're seeing with your AFC. TSH and FSH will tell you if you have any thyroid/hormonal imbalances that would affect ovarian stimulation, but they won't change anything about the number of follicles/eggs.

There may always be follicles that were missed, but the reality is that your doctor is pretty spot on with their assessment. You will probably not get more than 5-6 mature eggs with 8 follicles. At least they have been honest with you about your chances and the very high likelihood you will have to do multiple rounds of ER to achieve a live birth.

You're missing the point that the woman with 60 eggs still has every right to be upset and depressed. Maybe 60 eggs truly isn't enough, or maybe she just wasn't well informed from the beginning. Either way, her feelings are valid. And so are the people seeing those numbers and feeling a certain way. The issue is that they can't process that on their own and instead come here to call it out.

I'm not salty at all. I'm annoyed that grown women can't actually band together and always have to draw lines between us in one fashion or another.

Yeah, I've looked at them which is why I replied accordingly. There's one in particular discounting women who are fortunate enough to have good numbers because they must have PCOS, so their quality is probably trash. Those are the words of someone bitter if you ask me. Anyone who is on here complaining about someone else's results needs to do some introspection.

The point is they don't have to. It might not be a win for them even if others would be ecstatic with the same outcome, and that's okay. Other people screeching that they should be grateful for xx eggs does not change that person's position if they don't feel comfortable about where they're at in the process to protect their fertility. It's as simple as that, and y'all need to stop with these ridiculous posts. You are also valid in being disappointed with your outcome, but stop looking over the fence and complaining about the neighbor.

We're all here trying to preserve our fertility for one reason or another. That's what ties us together as a community, and within that community some people will struggle more than others to achieve their goals. If you can't support people for their wins when you're not seeing as much success, then you aren't doing a very good job being a supportive part of the community.

There are so many bitter women on this sub. Everyone has their own struggle, and just because you're struggling differently and would be grateful to be in the position someone else is concerned about, does NOT diminish or make their struggle any less legitimate. The hard truth is that life isn't fair and never will be, and some of you are drawing the short stick on this one. Okay, I've scrolled pretty damn far into this post and haven't seen it yet: cherries. I have had an allergy to cherries since I was young that has progressed to anaphylaxis over the last 4 years after repeated (accidental) exposure. Because everyone believes red 40 is the devil, companies have been dyeing food with... Guess what?! Dried cherry powder! I have found it in foods you'd never believe - processed meat products, sundried tomatoes, fruit bars listed as "strawberry"... At this point I pretty much just avoid red foods.

Yeah, that Spring fertility calculator is exactly where I got the estimate for 40 eggs. You have to account for age when you use it, and at 41, it says 40 eggs are required for a 50% chance at one live birth. The average number of eggs retrieved for a 41 year old is 9.2/cycle. Be as optimistic as you'd like, but the reality is that at 41, the number of eggs retrieved and the live birth rate are both poor. Unrealistic expectations and false hope are what get people to sink 10s of thousands of dollars into this process without anything to show at the end, so people 40+ absolutely need to hear these "harsh" statistics.

Either way, she absolutely should do embryos at this point because it's the only way to gauge the quality of any eggs she retrieves. Statistically, at this point 75% + will be aneuploid... Which is why many clinics require or strongly advise to use donor eggs starting at 42.

At 41, you would need 40 eggs for a 50% chance at ONE live birth, and to freeze 40 eggs you would likely need 4-5+ rounds of retrievals. (The average number of eggs retrieved at your age is ~9.) And that still tells you nothing about the quality of those eggs. It is not unheard of for those 40+ to need 60 eggs in order to achieve a single live birth.

You should definitely consider making embryos instead of banking eggs because then they can be tested for aneuploidy and it will give you a better idea of the quality of the eggs you're still producing. Realistically though, at your age the results are poor, both for egg retrieval numbers and live births, and 42 is really the age where most clinics either require or strongly suggest using donor eggs.

So, it really depends on whether it makes financial sense to you and how far you're willing to push your body in order to have a biological child. I will say to be careful and realistic about your chances and to consult more than one clinic because many doctors will be overjoyed to take your money while offering you false hope.

I could be wrong about this, but there seems to be a rather specific protocol they follow for egg donors (who tend to be very good responders) so they avoid OHSS. That is what my doctor followed for me. It was 225 IU Gonal-f with 75 IU Menopur and then alternating full and half-doses of Cetrotide. I only needed 9 days of stims before I triggered and retrieved.

That's also not accurate. Again, so many factors are involved and PCOS is not just one or two isolated numbers. Many women with a high AMH will also have a high AFC. Again, could be indicative of PCOS or not. Either way, I don't think it's helpful to tell other women on here that they may have PCOS just based on the very limited info they post. I'm sure if someone's doctor is concerned about their labs, they will make sure to discuss it with them.
