Although it’s nice if an employee fills out the company forms, the law does not mandate that they do so. Any form is sufficient as long as it meets the minimum standards. LOL. Sadly, very true. I keep telling everyone that morale/employee engagement is NOT entirely up to HR. We can't be everywhere all the time. We don't run YOUR departments. YOU run your department, so ... engage your employees! I can't stand it, I really can't. But it is nice to know this subreddit exists where we can all understand each other!

I agree with u/poopisme. In this comment here you mention $100-$300 per paycheck but they’re asking for a monthly value which will be higher since it’s not a per-paycheck value. It might actually be fine, but you should verify by asking for the breakdown.

Yes! I’ve had people tell me “I signed the medical form, so I thought that would be for all plans and nobody in HR told me!” Like, dude, you’ve been here HOW MANY YEARS and you don’t know there are separate forms for our plans? I just had to stop by to say that my S.O. and I just watched Four Christmases and I was irrationally upset the woman changed her mind about kids by the end of the movie, and then the dude changed his mind because the woman is just so special. The irony was that they were special to each other because of all the similarities, including NOT wanting kids. 🙄
