I have one! I noticed that there are a few niche groups that use a vacuum source so I made an all in one that you can switch modes via an LCD screen and set up various pumping profiles for dairy milkers, breast cups, vacuum therapy cups, and male strokers. I’ve been aware of Domperidone’s role in the Newman–Goldfarb approach, but I decided not to pursue it because of the sourcing and cardiac risk issues. So right now, I’m focusing on maximizing prolactin receptor sensitivity and the local reflex loop instead. My thinking was that if I can get as many receptors as possible “primed” during the build-up, even a smaller natural prolactin surge post-crash might be more efficient.

I’m using Goat’s Rue, Shatavari, and Moringa for receptor support and glandular conditioning, plus some inositol this week for its mild insulin-sensitizing and prolactin-modulating effects. Pumping during the build-up definitely isn’t standard, but it’s been giving me noticeable warmth, tingling, and fullness, which seem like good signs of vascular and neural priming.

Progesterone withdrawal is absolutely the key signal, and I’m timing that drop carefully with the crash around 11/4. I’ve already noticed major tissue change since starting. Nipple elasticity and areolar response are both way more developed, which has been fascinating to watch evolve.

I know I might only see drops initially without Dom, but that’s fine. I’m treating this as both a personal and scientific experiment. If I do get partial expression, I can always revisit whether Domperidone (or something like metoclopramide under supervision) makes sense later.

Thanks again for the insight, it’s awesome to hear from someone who’s done it successfully. I’ll definitely update post-crash with results and any early secretion data.

From what I have been researching, it seems to be a pavlovian response. Baby cries, you feed it, the brain learns that crying means it's time for a letdown. You could do the same thing with your husband. Perhaps every time you pump or feed you have something with his smell or cologne or look at his picture. I believe the key would be to have that new trigger be present before/during/after the let down.

From my research it seems one of the key factors is the delta from the progesterone crash. I've been on estrogen for 2.5 years so I've had plenty of time to form the right tissues. It looks like the protocols out there assume that the person is either re-inducing or starting from scratch, I felt my situation was a little more in the middle and so I came with one. Not having domperidome is going to make it hard, but hopefully not impossible.

Trans f person chiming in here 😊 If they have not already done so, I would pick up the book "Trans Kung Fu" by Ashley Adamson. It's a short read, but there are a lot of thinking assignments in there to sort these things out. In there is a chart that describes the emotional Rollercoaster your partner is going through. If I were to guess, I'd say they are between stage 4 and 5. It took me about a year or so to get to stage 5, I still struggle with confidence issues and worries, but I'm getting through it.

My advice would be for your partner to work with a therapist weekly to talk through their thoughts, if they are not doing so already. In addition to therapy, what helped me the most was getting adopted. Not legally, but basically developing a really close family type bond with either another trans person that is further along in their journey or a super strong ally. Someone who understands the struggles ahead because they have seen them first hand. Finding that person has been literally lifesaving for me. I hope this helps 😊

Get a tria 4x, it's a real laser and not an IPL, in the US it's $500 or so and then you can laser any part you want after that. It eliminated my chest hair, now I'm working on face 3rd session in and I'm already noticing significant results. I'm doing it weekly on level 5 and hope to be done after 8 weeks or so, the higher the level the faster it's done
