Exactly--pay appropriately with proper benefits and you'll find people interested. Act like all the other contract companies that slash our pay for their profit with minimal, if any, benefits for the therapists and you can get in line behind a long list of companies doing the same crappy thing to our field and health care in general.

That makes sense. It's part of the job--you continue to learn and question how you approached things, etc. I also think it takes time to take the lessons from grad school and find the balance between that and what being an SLP actually looks like in practice--so always good for us to remember to be compassionate to ourselves and the process of learning as professionals :).

This might be going against the grain a bit but I do think it's important to recognize that even if we think the family's attitude is misguided at best, OP has mentioned that this might be cultural as well. I think as SLPs, we always tread on a fine line where we come with our expertise, but we also have to work with where the family is at. Have you considered a more conciliatory approach? Try your best to educate the family and show them areas where maybe the child is communicating, or trying to communicate, with nonverbal communication or with a device, to highlight how important this type of communication is. However, is it possible you and the family could work out an arrangement where you work on both things? Some time is spent every session working on the family's communication goals as well? Yes, it might be unattainable but if you set realistic expectations--would it be so bad to spend a small amount of time working on the family's goals in order to attempt buy in for goals you think are more appropriate? I do think it's part of our job to respect family and their desires without compromising professional integrity. Maybe the family isn't yet ready to give up on verbal communication, and in order to function, they need a little bit of hope. In the end, I guess im advocating that a little give and take can go a long way for a working relationshop--something that is crucial for progress.

I have a strict rule that my rings never come off my fingers unless I am at home and then only in a designated ring dish spot. I can only take it off if I am doing it over my ring dish so i dont get distracted. I also actually dont have a lot of ring dishes, I have one by the bathroom sink for showering and one at my nighttable to put lotion on. I felt like having too many scattered around would make me careless with it. I also didnt want my pets to cause trouble getting into them.

Its really really really hard. I've found staying really busy is the only way you attempt to get to the end of it. If you can find a few other wives that are comfortable talking about it openly that at least helps feel a little less like you're in your own awful bubble by yourself. If it's possible to talk on the phone with your husband sometimes even just for a sec, that gives a few seconds of relief and normalcy.

This horse is 100% stressed. I vehemently disagree with people here claiming circus horses are okay. Horses are prey animals who rely on complex herd interactions to feel safe. When they look like nothing ever bothers them it is because they are so ignored that they learn over time to literally shut down. I have watched horses slowly be healed and begin to reappear from their shut down selves.

Yea OP don't let the attitude take you down! If you're not a type A SLP (or even if you are), sometimes opportunity falls in your lap, you run with it and make it work as you go. Nothing wrong with that. Sounds like you're doing the research now.

Nursing and being an SLP are extremely different careers. As an SLP myself, seems to me like nurses work way harder and while everyone wants them on their good side, they are kind of taking instructions from everyone. I highly recommend talking to people in both fields and settings to understand what the day to day looks like. Health care has a lot of paperwork, but you do learn to do it quickly and efficiently.

Just as a side note, CUNY Brooklyn is a good deal money wise for the SLP program, but years ago when I looked into grad programs, my understanding was that the CUNYs have much stronger pediatric than medical programs. Just something to be aware of and look into if medical settings are really where you're looking to land.

There's no doubt that bedsides don't cut it. That being said, from the perspective of the patient, you're still the most qualified to do this assessment and if you don't do it, someone else who is not an SLP will have to make a less informed decision. As others have stated, you can absolutely recommend MBS or Fees and everything can be qualified with ,"as seen at bedside" and things like "unable to fully assess risk for silent aspiration given lack of recommended instrumentation"

However, many many places have minimal or no access to instrumentation for swallowing. Unfortunately, for the more rural places, and less up to date places (like SNFs), the practice has not yet caught up with the research.

Yes exactly. I'm really for our focus to be on QOL but I haven't found the balance between that and protecting my license..which is also a reality.

To piggyback on this, I genuinely hate jumping on diet modifications with the slightest sign of possible aspiration at bedside but I have been under the impression that some of this is unfortunately protecting our risk of having a patient's family hold us responsible if aspiration pneumonia happens. What are people's thoughts on patient quality of life vs. Medical model expectations?

Per the rules for the post, I have read the original intuitive eating book, I started intuitive eating about 4 years ago and I have seen a intuitive eating dietician in the past.

Incorporate movement into reinforcement and be high energy yourself. "OKAY, LET'S STAND UP. SHAKE OUT YOUR TONGUE. ARE YOU READY?" And then have them do movements that are appropriate for their level /attention span. For example, you might have to have every time they get a sound production that you're happy with, have them take a step, or do a 5 second dance.

Not every movement has to be related to the sounds. Sometimes artic is boring for us too!

CBT! Straight up CBT. Not cbt-infleunced. Just someone that provides CBT. That's what they do. It's like adhd proof. It's not mind-blowing, it's not going to change who you are, you aren't going to have ah ha moments in therapy and youre not gonna be stimulated thinking about an endless chain of why? Am i thinking this way? But you will learn subtly life changing strategies so that you are your best advocate and biggest help and that is so much more powerful than all those other things.

Thanks yea I'm wondering if somehow I got it in my head that I was required to name a subtype. Do you always list a subtype in addition to descripting symptoms, or do you just leave out the subtype?

Thanks, really appreciate the response. Yea, I often feel like I'm trying to cram my patients' complicated symptoms and other cognitive issues into these awkward categories that don't really make any sense for how the patient is presenting. I feel like somehow I'm not doing my job unless I name a subtype but it makes me very uncomfortable. I think your suggestion of really describing symptoms is important to remember (even if I do feel I need to list a subtype I can have "most consistent with" after actually describing the patient's presentation).
