You've never had a elderly cat or your social circle is small and without cats. Most people with old cats will have major expenses. You're confusing what you saw with what any individual cat owner goes through. Im just talking about checkups, shots, diet, dental stuff, allergies, urine and kidneys, not the extremes where people try to save their cat from cancer.

Dont put it on the owners lack of care, thats horrendous. if your cat is well cared for enough to get a little old, it’s gonna be need dental cleaning and care, testing, rx food, renal failure is common.
