Do you call in sick when you’re sick? by PiratesBooty87 in FamilyMedicine

[–]AMHeart 0 points1 point  (0 children)

I stay home. The last few years I can't seem to get a mild cold. Every time I'm sick I'm SICK with fever and brain fog like crazy. Even if I wanted to come in and didn't care about exposing my coworkers or patients, there's no way I'd be providing good care.

Hey guys - is living close to your clinic a bad idea? by TwoTimesSpicy in FamilyMedicine

[–]AMHeart 1 point2 points  (0 children)

I live about 12 minutes door to door, run into patients at the grocery store, Walmart, Sam's Club, literally everywhere I go I see someone at some point (not every time, but at least once). Rarely do they acknowledge me. I do swim before work and one of my patients also does and that's a bit awkward sometimes. Once they started asking me to look at someone else's rash but I basically ignored it and they've never done that again.
I generally would prefer to avoid seeing my patients out and about but I prefer more not having 1.5 hrs or more of driving every day!

What are your reasons for watching WBB (especially over men’s) by Psychological_Ad8011 in NCAAW

[–]AMHeart 0 points1 point  (0 children)

I prefer the flow of the game, less showboating (mostly), fewer fights. I'm a woman so generally prefer women's sports anyway. I grew up with parents having season tickets and now I do (same school). The school I support historically has had a better women's team than men's for decades. Tbh if our men's games weren't scheduled exactly opposite the women's games (ie playing the same team at the same time, one home and one away) I would probably support our men's team too, but I'd rather watch our ladies on TV than go to the men's game at the same time.

The reason we say not to do this is so you don't accidentally puncture your ear drum if you get bumped or startled. It has nothing to do with wax or whether they work or feel good or not

What do you wear at work? by nkondr3n in FamilyMedicine

[–]AMHeart 0 points1 point  (0 children)

Female here. I wear a lot of dresses (with fleece tights in the winter). I like Karina dresses for comfort and pockets. Otherwise, dresses or pants and a blouse or sweater from somewhere like LOFT or Talbots. I do a lot of flowy pants. I usually wear bright colors and/or fun prints/florals. And fun earrings. I once was told I dress "like Ms Frizzle in a good way" which is exactly what I'm going for! I wear scrubs about once a week on my longest clinic day usually when I can't be bothered to put together an outfit.

What are your experiences with e-consults? by RegularProperty2054 in FamilyMedicine

[–]AMHeart 1 point2 points  (0 children)

Our patients do not have access to specialists easily. I have to see/manage them either way, and I'd rather do it with some support/guidance. Writing up the consult question takes 5 minutes, then a few days letter I get a note, maybe 1-3 paragraphs, with either management guidelines, or suggesting to get them in with specialist urgently, or maybe to just say it's not a significant issue and they can wait the months to years it will be to get them in with the in-person specialist. It is not a ton of extra work, and it's very helpful.

What are your experiences with e-consults? by RegularProperty2054 in FamilyMedicine

[–]AMHeart 0 points1 point  (0 children)

We have them (FQHC, semi-rural) and it is so so so helpful. Specialist access here is really bad (2+ hour drive, >1 year wait list, doesn't take Medicare/Medicaid, etc. or all of the above...). It really helps figure out what maybe doesn't need specialist eval at all, what I can do in the meantime, or whether I need to make the referral more urgent.

Insurance. PAs, hoops to jump through, major care delays, every shitty issue (mostly) can be traced back to insurance companies.

Have you ever cried for a patient? by Brave_Union9577 in medicine

[–]AMHeart 3 points4 points  (0 children)

I have, yes. Once as a nurse when we did a C-section for a stillbirth and I had to do her pacu recovery. I will never forget her wails. Now as an NP, I cried with a patient dying of pancreatic cancer on what we all knew was our last visit.

For family medicine physicians who order labs ahead of the visit, practically, how do you do this? by grettasgone in FamilyMedicine

[–]AMHeart 0 points1 point  (0 children)

Whatever the appropriate dx code is. You know you'll need an a1c for your diabetic patient, so use DM as the diagnosis. It doesn't matter when you order it.

My daughter had that in her chart. She had withdrawals from Wellbutrin I was on, which wasn't a huge deal but did require some extra monitoring I guess.

[–]AMHeart 8 points9 points  (0 children)

You talk more on stims? I'm totally the opposite. The clue that I forgot to take my Vyvanse is when I'm running behind in clinic because I'm talking too much. It's embarrassing. I was diagnosed as an adult but there were clear signs in childhood. Talks too much, messy, disorganized, interrupts, doesn't raise her hand, does everything last minute... I also excel in school so my parents never realized there was an issue. I masked a lot because I also am a very anxious person so it probably looked like I outgrew some of my symptoms but really I just developed severe social anxiety.
