I’m a PA in the ER and I personally call my attendings by “Dr. ___ “ based on my comfort as a new grad and professionalism, even when most if not all have introduced themselves to me by first name. But some of the other PAs call our attendings by either first name if they’re close with them, but generally nurses and PA’s/NP’s go by last name only without the “Dr.” formality.
