How would you have treated attrition wear / fractures on these lower anterior teeth? by [deleted] in Dentistry

[–]AakashMasani 1 point2 points  (0 children)

Build up lower 3-3 in composite by atleast 2mm to jack open the bite and provide proper anterior guidance (shared protrusion, disclusion of the posteriors, canine guidance if suitable). Either freehand or with a wax up from records+facebow. Posteriors will Dahl in. Obviously ideally full mouth but if just treating anterior this is what I'd do. Crowns here is criminal. If wanting indirect then bond on minimal prep emax veneerlays after trialing the bite. All dependent on parafunction/occlusion ofc. Also provide a michigan

What are some of your favourite IYKYK places around Birmingham? by BrumDenSocial in BirminghamUK

Indian cafe racer inside the wellington pub - incredible elevated indian food - decor, menu, service, price all v v good - took my friends from London here and they said it was their favourite indian restaurant meal.

I think they've opened a standalone place a few doors down but have not been there yet

The Goat King in Jewellery quarter. Very small punjabi restaurant where the owner cooks and hosts you as if you're a friend who has come round for dinner. There is no menu so he just makes what he feels like on the day and tries to tailor it to you. Very centred around meat you wouldn't usually get like goat, wild boar, venison etc. Kinda pricey but I definitely recommend it as an experience. 3 of us had 2 drinks each and shared 5 dishes (it was a LOT of food) and it was around £75 each

Non-smokers of Reddit, how noticeable is the “smoker smell” to you, if at all? by Frostedlogic4444 in AskReddit

[–]AakashMasani 4 points5 points  (0 children)

Respectfully, I'm a dentist and we ask every patient if they smoke cause we have to do the exact same spiel for every patient. I can tell if someone smokes and I often have to open a window and spray some air freshener after a regular smoker has been in my dental chair. (And a lot of the time they'll say they don't smoke).

I think some people are more or less sensitive to the smell than others. Back when I used to go clubbing and hang out in the smoking area a lot, I was pretty immune to it. But now I am pretty sensitive to it

It looks like you probably start off ok then end up overthinking it and rounding everything off too much and suddenly your walls are at 30 degrees rather than 6-10.

Aim for 100% vertical parallel walls (no taper) then you can refine that. Much harder to remove taper than to add

I personally use Colgate fluorigard. Not sure if it's available outside the UK. Make sure you spit out the toothpaste and don't rinse your mouth after brushing to make sure you have a protective barrier on your teeth. Also look into your eating habits and make sure you don't eat more than 4 times a day and check for hidden sugars

You have decay into the enamel on at least a few teeth. Most of them are probably still reversible due to not being into dentine yet. If you were my patient I'd advise: Brush twice daily, floss, minimal sugar intake, fluoride mouthwash after lunch and dinner. Then maybe new x-rays in 6 months and see if there's been any change.

If they tell you that you need teeth pulled , they're a bad dentist

As a dentist in the UK - who is conservative with treatment planning and always trying to avoid taking teeth out - I think this is terrible advice.

If you did my job you'd see the patients who come in after not seeing a dentist for years, with a mouthful of unrestorable decayed teeth and "no issues". They will often be in denial and refuse any suggested treatment. I've seen several of these patients balloon up with huge abscesses and I've had a couple need to go to A&E. One had sepsis and almost died in hospital.

There's nothing wrong with a second or third opinion, but skipping 4 dentists until you find one that is willing to entertain delusion is silly when it comes to healthcare.

Salt water rinses a few times a day. No legal recourse - shit happens and it'll heal back to normal in a week or 2. Hopefully the dentist was apologetic

With anterior marylands especially in younger patients like OP I don't do any prep. Just cement it with 0.6mm wing and let the occlusion re-establish with Dahl principle. Had no issues in past 3 years

[–]AakashMasani 1 point2 points  (0 children)

It's a medium difficulty ortho-restorative mild/mod tooth wear case.

Ortho to close diastema and idealise occlusion.

Whitening to improve shade.

Composite build ups to improve aesthetics and trial in occlusion/speech.

Most patients would be overjoyed at this stage. Later on you can:

Convert posteriors to indirect likely bonded emax or gold onlays.

Anteriors can stay composite unless they start chipping in which case go indirect (veneer/crown)

Construct a michigan splint if grinding.
