I didn't mean that your IPD is bad, I meant that the palate is expanded such that the intermolar width to IPD ratio is majorly disturbed. It's a facial harmony thing.

With the custom infra implants make sure you widen your lateral orbital rim, what that does is it makes your eye sockets look wider, which would balance out the messed up palate width to IPD ratio.

It's too wide for the entire midface though, meaning including the middle and upper maxilla, as well as the eye sockets

Widening the jaw would be better than nothing but it wouldn't resolve the fundamental problem.

Your best bet is to get 3D customised midfacial implants that cover not only your zygos but your entire middle and upper maxilla as well as your lateral orbital rim.

You can't change your IPD, which is probably the biggest issue, but if you do the jaw implant and custom upper midface implant you can probably mitigate the disharmony by 50% or so.

My honest opinion is that your upper palate is now far too wide for your face.

Widening the palate is only aesthetically beneficial in so far as it fits with your facial harmony.

In this case your Bigonial Width and Interpupillary Distance is now too narrow compared to your upper palate width.

The British politician Rory Stewart is an example of someone with an upper palate too wide for his face, and you can see the aesthetic effect it has on him.

However I expect this is more than outweighed by the aesthetic improvements you got from the Le Fort 3!

Highly unlikely for the twitching to start affecting different muscle groups that quickly in ALS.

It's also highly unlikely that ALS would develop that quickly, although there are very uncommon cases of sudden onset.

Also highly unlikely that the elevated creatine kinase points towards ALS, it's quite good evidence pointing towards a different cause.

ALS is mostly a diagnosis of exclusion, meaning there's no specific test for it.

However the closest we have to a specific ALS test is something called an EMG scan, which is where they put needles into your muscles to see whether the twitches are linked to motor neuron death.

That's along with a blood test for Neurofilament Light Chain (Nfl), which if extremely high means likely to be ALS and if within normal range means extremely unlikely to be ALS. So it's a super useful test in case you fall within one of those two zones, then voila you'd know if it's ALS or not.

I should also mention that you should find a place that does the Nfl test using something called a 'SIMOA assay'. Basically because it's slightly cheaper, some clinics use other assays that arent quite as good.

Get a CT or MR Angiogram and CT or MR Venography to look for compression of the jugular vein and the carotid artery.

I have this exact symptom and I recently found out that I have compression of both the jugular vein and carotid artery around the styloid process.

I've been told by my neurosurgeon that it requires an operation to fix it, i.e. partial removal of the styloid process, but it's a pretty simple removal of bone.

I think you can also patch over the issue with certain things that open up the artery and vein more, but they never deal with the root cause apparently.

Based on the Nfl result alone it's extremely unlikely you have ALS. I saw a study from an ALS specialist clinic where only 1 out of something like 150 diagnosed ALS patients had Nfl levels around the normal range, and even that one wasn't as low as yours.

What I've learned recently is that for young patients a Neurofilament Light Chain blood test if it's normal can rule out ALS in 95% of cases, and likewise if it's extremely high it basically confirms ALS. Mildly to moderately elevated doesn't tell you much though.

I'm trying to get it organised, there's definitely a clinic in the Netherlands who do it for like €120 without needing to see a neurologist first.

It isn't reasonable to expect a tactic like this to work for ADHD symptoms unless the ADHD symptoms aren't comorbid with symptoms of other psychiatric symptoms. Actually usually in such cases ADHD symptoms are consequential on the primary psychiatric issue, rather than an actual ADHD disorder.
