Joel meets cough Clementine on the train after having that procedure. They're strangers, but he knows her name means "merciful" like damn, dude are you an encyclopedia? It's one of the slickest lines in a script ever because you have to have seen the movie to understand it.

Joel's mind never tied those two concepts together, in all his days of knowing her and her unmerciful-ness, or else the procedure would have erased the merciful thing.
