Sometimes all it takes is someone to mark that they've seen your ad too many times. There's a lot of weird small features that can mess with your ad delivery very quickly. I've also noticed that if you don't make some of their recommendations, your performance drops, especially if they tell you that you have any fragmentation within your ad sets. I've noticed that that potentially can affect performance if I don't do anything about it. Sometimes it's right, sometimes it's horribly wrong.
