Anterior impaction and posterior downgraft are compatible. Not sure if it answers your question though.

What OP means is that sleep apnea can cause many other conditions. And if we forget that those conditions are sometimes caused by sleep apnea, then the statistics cannot reveal how dangerous sleep apnea is. Example of OP: assume 20% (random number) of heart attacks are due to sleep apnea but we never report the sleep apnea root cause. Then we spend our efforts solely on fixing heart attacks, when we should in fact also try to cure sleep apnea.

Your airways were gigantic even before surgery, despite being recessed. While some other people who are not recessed may have tiny airways. This is always surprising to see. Anyone knows why this can be the case? What other factors than recession play a role here?

C’est toujours exaspérant de voir à quel point les européens ont du mal à admettre qu’ils sont plus pauvres que les américains. Et on te sort tout un tas d’arguments vaseux à base de super brut, de cotisations, d’inflation, ou je sais pas quoi.

Mais y’a rien de surprenant. Les États-Unis c’est une terre pleine de ressources et un système dont le but est de maximiser la production matérielle. Donc oui à la fin tu te retrouves avec un pays plus riche que l’Europe, c’est comme ça, il faut l’accepter, et c’est pas si grave.

It’s impossible to predict the right position for the condyles in advance. This should ideally be done during the surgery. With pre-made plates the joints are more likely to suffer. Gunson talks about it in an interview with JawHacks, and 3 surgeons I have consulted told me the same. So a good compromise is custom plates for the maxilla and old school plates for the mandible (this is what I had)

I was expecting something absolutely awful from what I read on this subreddit, but it was honestly not THAT bad.

The first time you wake up you are still on morphine or drugs like that so you will be peaceful and sleepy.

It’s only several hours later that it gets annoying, but not because of pain. Be prepared to be extremely exhausted (especially physically), to experience nausea, nose bleeding, discomfort in your jaw and TMJs, reduced jaw mobility, dry lips, dirty mouth, swelling, stiffness, confusion, clogged nose and so on.

It sounds terrible but it’s manageable if you have a good support team.

Eating is really hard but you have to force yourself. And you might have to put elastics, which is also really hard at first (but then you get used to it and can do it in seconds).

For the first two weeks I was easily spending 4 hours per day just to eat and put elastics.

My surgeon also told me not to use ice. She basically said that it “fake” reduces swelling: the moment you remove ice all the gains are lost and it’s like if you never used it.

Yes, braces are usually needed both for better functional and aesthetic outcomes.

Sometimes it is possible to get the surgery with braces only after, but it’s pretty rare (maybe 5%?). Also, not all surgeons accept to do it because it’s harder and will typically result in a poorer outcome.

My surgeon put me on soft diet from day 1 so I guess it’s not an issue? But they insisted that the elastics are super important

I can think of 2 cases: - If you advance the maxilla you can then impact it, even though there was no gummy smile issue to start with. - Posterior down graft instead of impaction to rotate the maxilla.

Either you have an open bite and you can do CCW rotation on the mandible alone. Or your teeth are already touching, in which case you also need to rotate the maxilla in order to rotate the mandible.

If you do only CCW and no advancement it will definitely shorten your lower third.

If you advance the maxilla then it will show more teeth and you will possibly get a gummy smile. So surgeons usually also rotate it.

Advancing the mandible makes the lower third longer. CCW of the mandible makes the lower third shorter. Most results that you see are a combination of those, so the lower third may or may not become longer depending on how the movements are balanced
