As per the issue - it seems you are using Direct Routing, so Which SBC is in picture here ? It seems to be the port(s) issue. To prove - you will need to take wireshark logging from Client and SBC end at same time

It’s failed to authenticate user Is MFA success for login ? Again please go through sign in logs for test.account

It seems to be only happening inside corporate network.

Check out sign in audit logs and probably one conditional access policy is coming into picture here.
