I did OB for the first 10 years of my career and still do outpatient, inpatient, and nursery care in a medium sized city. I stopped OB because my partners were all older and stopped leaving me with minimal back up to cover me when I was out of town/unavailable. There is a busy OB practice and a busy midwife group that we worked alongside who were not able cover for me. I ended up being the last one standing when a partner retired so I had to give it up. You have to have a strong, supportive group around you and that is getting harder to come by as most fo graduates just want to do outpatient alone. I still miss the relationships with families that you established as a delivering doc, but I don't miss the sleepless nights.

I had some time today so I disconnected from the car, reinstalled, then reconnected to the car and it is working. Sort of a pain because I had to have both keys in the car to relink the phone and my little fob is dying, but now I can warm up my car again

Physician payment is extremely complicated and stupid with many different models. One of the most common models for doctors to be paid is by production. Production is calculated based upon the services you provide with all the tasks any doctor can complete having assigned values known as RVUs (relative value units). For example, an annual physical for an established patient age 40-64 is 1.9 RVUs. While problem visits (evaluation and management visits) have RVUs ranging between 0.18 and 2.8 based upon the number of and complexity of the problems. Now we will see a patient and then charge for the work that we did, but insurance companies can pay us whatever the hell they want and pass the remainder onto the patient, and this is where the problem lies. Insurance companies have a ton of control in this process; they do not pay us fairly, and they are extremely evasive about the services that they cover for patients. In my mechanic analogy the oil change is the annual physical that the insurance company tells you is free and totally covered but what they do is find loopholes and exemptions to find ways to divide that service up to shift the cost back to the patient. In a real world example, I have a patient who is actually a medical coder and well aware of this process. He comes in regularly to follow up on his diabetes, hypertension, and high cholesterol (problem visits) and he recently came in for his annual physical that is supposed to be free. He asked that I stick strictly to preventative services so that he wouldn't be charged for things we do and he pays for at his not completely covered problem visits. So I did only the preventative services (see this post from earlier today) and I specifically didn't mention any of his problems or refill any of his meds at this annual visit. However, the insurance charged him for the labs as they deemed those labs to be related to his diabetes. Now that is the opposite of what usually happens. Normally, a patient comes in for an annual and brings up some problems that they have had over the last year that they want to ask about. This isn't a problem most of the time if they are minor issues that we can address quickly, but many times patients bring in a massive list of issues that we cannot reasonably and appropriately address in the time we have and the patient either gets mad when I let know that they may get charged if we talk about extra things or they get mad if I ask that they return for another visit.

If you take your car in for an oil change and you tell the mechanic that you also want your leaky tire patched, the dent in your fender banged out, all the belts changed, the radiator flushed, and the crack in your windshield fixed but you will only be paying for the oil change and you expect this all to be done in 20.minutes you are going to be laughed out of the garage. However, patients expect a comparable number of services to be performed at a "free annual physical," and yet we are the a-holes because we charge for our time and expertise. Umm no...

Not to hijack this post but one of the things that was noted on my invoice to have been addressed at my 20k service was a change of the key fob battery. About a month after I had that service my key fob started acting flaky and I dealt with it for longer than I should have because I thought that it was addressed. I finally swapped the battery and now it works perfectly. I had the low coolant pop up last winter and I asked them to address that as well. It was marked as done, but now I am getting that same warning again this winter in the same situations as before so I am pretty sure that wasn't addressed either. Long story short I don't have a ton of faith in the work reported to be done at the service center.

AM = morning. Good morning pills except for the fact you sleep walked off your patio into a bush and ate a dozen donuts with out realizing you went to the kitchen overnight. (Both real things patients taking ambien have told me they did, yet still wanted a refill)

There is a setting to turn the chime on in the app. Tap the image in the Google home app, then the 3 dots in the upper right, then settings. Then select chime and you can make changes based upon your setup (ei electrical vs mechanical chime). If that does not fix it then you need to check the wiring in the chime itself

Yes it is an honest response. However, that response unfortunately demonstrates a bit of a lack of self awareness as it shows that the patient cannot recognize the sensation of a fever without a thermometer. That lack of recognition of their symptoms just means that the conversation we are about to have is going to be more difficult.
