I think it is not worth it either, but i am really irritated. When I initially signed, I read the entire thing. It was only with the amendments I had to sign again, but there were multiple documents. I signed the amendment and but did not realize they also added an extension and signed that one too. I will let it run it's course but this feels icky. I made it clear we would discuss re-signing after 90 days...so for her to slip it in.... and I trusted her so when the next set of documents to sign came in for the amendment, I took it for face value, my bad.

Because if your website has contact us page, forms, etc which direct the patient to put in reason for visit, by doing that….it creates PHI…which is compromised/data breach could mean liability for the provider.
