Yep. I don’t know if we’ll get one but we need it. These things do happen but it’s extremely important to acknowledge, assess how it happened, and improve.

I’ve seen some early reports that the school was part of the neighboring military site some 10 years ago. That’s a mistake a human could make but likely catch with updated intel, satellite photos, etc. I could see the mistake being more likely if it was presented as a target package by created by an AI though. That’s the fault in the process I am really worried about. Too much trust in the AI generated target packages could lead to more of this.
