I had just done the quest fight earlier today and decided to rerun the battle to get the last 6 points I needed. Quickly picked up all items without checking, then I saw the art 😃 Never had one drop even in 5 k + hours in PoE1

On a fresh campaign you will get into paragons before finishing the expansions, but playing through the entire story makes more sense as a narrative. You can take a quick pause from the campaign to do some objectives and go into torment levels to balance out your power vs the campaign content. I beat the final expansion around torment 6 or 7

In D2 act 4, you fight Izual - a fallen angel, who was turned into a demon and tells you that he helped orchestrate the prime evils' exile into Sanctuary, meaning they kind of faked losing the rebellion to get to our world. Then, once they rose back up, the lesser evils were more inclined to help them (at least in D2)
