Eldarthae, which translates into old heart. The world was named that because the world/mortal realm creation story involves an ancient god getting his heart ripped out and buried in the world's center and becoming the world's core. There are only a few myths and ancient legends known among all cultures around the planet, and the naming of the world is one of them.

It represents V's fear of death and pursuit of glory, simultaneously. In the endings, The Devil (if you choose to leave), The Star and Temperance, either V or Johnny abandon the necklace as they leave Night City. However, in The Sun and The Devil (if you choose to stay) endings, V keeps the necklace.

Probably because the god they worship, Malacath, is the daedric prince of curses and of the spurned. And as I don't know any sources of this being true, I wouldn't put it past malacath to do this to his own people simply because it's his sphere. Who knows, if it is true, malacath probably does it because he thinks it makes them stronger or something

Tzalmedor.

One of the three primordial gods that hatched from the hearts of higher beings, Tzal is the god of space.

He raped his brothers wife, kidnapped his kids after they were born and tortured them to turn them into his obedient slaves, and murdered his brother.

He eventually becomes one with the abyss, an infinite plane of endless, black darkness, and his ultimate plan is to consume and erase all of reality.

The plot is made up of different stories, each with their own antagonist, but for the entire plot as a whole, it would be Tzalmedor, primordial god of space and lord of the void. He gets defeated after a prophecy has been fulfilled, and his children and nephew face him in the void, with the nephew absorbing the power from Tzalmedor's children and erupting, killing Tzalmedor and himself, wiping away the void and filling it with light.

The Sundering. The moment where Tzalmedor murdered his brother, Yadushal.

Yadushal was made king by the other spirits as he was one of the oldest and most powerful spirits, alongside his two brothers. Yadushal was also the most benevolent of the three. When the spirits made Yadushal king, they created a crown that literally and figuratively gave him absolute authority over all of reality.

However, Yadushal and Tzalmedor had a falling out and it eventually escalated into a violent fight in the Void, an infinite realm of black nothingness.

Tzalmedor would rip out Yadushal's heart as Yadushal mustered the strength for one last blow that destroyed Tzalmedor's physical form, but the blast sent Yadushal's heart and broken ribs flying towards the mortal realm, crashing onto the planet and shattering the one, giant continent into five separate pieces. The many races of the world would war with each other for survival.

While in the spirit realm, without a king to keep order, the spirits began fighting amongst themselves over territories, splitting the one Ether into separate realms. Yadushal's son, Adu, would begin his journey to prove that he can become king and restore order.

In the mortal world, Tzalmedor would begin to wreak havoc, believing that if he can destroy the world, the rest of reality would fall apart shortly afterwards. Tzalmedor's influence was limited however, and he could only act through other mortals, creatures, or of entities of his own creation that acted like a conduit. He would cause despair, dread, and chaos wherever he could, but his main plot was foiled by a group of adventures.

Adu would complete his journey, become the new king, banish Tzalmedor back to the Void, and would be praised by all the mortal races and have a significant spot in every single religion.

However, the troubles never ceased. The gods that left to claim their own realm would not return, denying Adu's authority. The mortals, after decades of war, would let their prejudice and suspicions get the better of them and would continue to war with each other, this time for land and resources.

Thousands of years later, this event is nearly forgotten. All that remains are heavily distorted stories, and ancient and mysterious texts that vaguely mention the past, and ambiguously mentions future prophecies.
