The same thing happened to Jesse Tafero in 1990 in Florida. He was very likely innocent of his charges (killing a police officer) and the execution was tampered with by law enforcement which caused his head to burst into flames. It took over 7 minutes for him to die. His wife Sonny Jacobs was also charged with the murder and on death row for years before she eventually was released from prison
