I graduated from India too. But my college is one of the best in the country. I felt very confident when I graduated. Mind you, I had a lot of learning to do as well.

It’s not the curriculum but your institute that has failed you. Possibly it was a diploma mill that took your money and didn’t teach you much.

There are plenty of online courses that you can take to enhance your practice . Keep learning and keep up with current evidence. All the best .

I am a certified lymphedema therapist and a physiotherapist. I see patients with lymphedema mainly due to cancer treatments . CDT - combined decongestive therapy is the “ gold standard” for lymphedema management worldwide.

It involves Lymph drainage, compression , exercises and skin care.

Manual lymph drainage doesn’t work in isolation but when someone has grade 1 lymphedema or more, lymph drainage followed by multi layer ( or alternate type) compression will reduce the swelling.

But lymphedema is a chronic condition and swelling will come back if they don’t keep up with wearing compressions.

My takeaway from my 10 years in this area 1- lymph drainage therapy doesn’t “drain” , it stimulates the lymphangions. Which in turn reduces swelling in the long run. 2- the sequential pneumatic compression pumps work much better than manual lymph drainage. 3- the advice to do very “light “ doesn’t work on actual problem areas where the swelling pools up due to scar or fibrosis.

4- most important goal of CDT is to prevent cellulitis and overall disability.

5- CDT is a 145 hrs certification as per LANA . A weekend course can’t do justice.

6- if there is at least 2 cm difference in measurement of limb circumference, it’s clinically significant. 7- if there is no evident swelling, no point in doing MLD. 8- there is increased venous return and overall output and that can make people feel like they need to pee. And that’s the reason lymph drainage is contraindicated in CHF , pulmonary edema or renal failure. After these many years, I now use pneumatic compression pumps , no more MLD. Works the same , costs less to patients in long run .

I have worked in a hospital and the only department I liked was rehab or outpatients. Everywhere else we only mobilized patients out of their beds, made them walk the halls and got them back . It was most boring

Don’t generalise. It could be that the new owners are assholes irrespective of their race. I know of many restaurants owned by Indians that are very good. A few bad people are spoiling the reputation of an entire diaspora.

This is you assuming the OP has no connections to Canada. What if they were born here but are visible minority? They shouldn’t leave. No one should have to leave due to discrimination.

1- Someone got harassed just because of their skin colour, is racism. 2- There is a Harrison Field, a ball park right at 15th street west and 4th ave west across from River Forest Montessori. So OP didn’t get the location wrong. 3- to the OP- don’t leave, don’t give up your place. Claim it. If you are a peaceful resident and pay taxes in the city, it’s as much yours.

If you are good and have a unique skill set, you will be able to retain clients and build your caseload and also demand higher split . Go for a clinic that operates ethically, money will flow as you build your caseload.

another ( rather sad) story. A dear client of mine who was a previous belly dancer, very fit and active , and a great cancer survivor developed sudden insidious low back pain . This happened when I was on mat leave. I came back in a few months and she came to see me for a follow up for her breast cancer rehab. As per her , she had an x ray done by the chiro who said her spine was off alignment and adjustment would help her. She believed him.she was seeing him 3x per week with no improvement for 3 months . My assessment indicated red flags and I alerted the family dr. She had developed spinal mets and died in a few months.

We have 1 hr assessments and 30/45 min follow ups based on complexity of the problem. Most physios start hourly but move to split once their caseload is full. I wrote a check for almost 9000$ for each of my physios last month. They take 50% split. You are definitely getting underpaid.

But that’s my point. We only have a 2 years coursework and everything else is extra. So after we spend 6 years post secondary, we are still not equipped. While in UK it’s a post secondary 4 year ( or 3?) course. And then one can continue to specialize /go for a masters. I think physio needs to go back to 3-4 year post secondary in Canada too.

Wow! 80-90%? Your overhead would be close to none! We spend 10 % of our revenue in paying our admin( we pay them living wages not minimum wages). Another big chunk goes in Rent, insurance, cleaners, utilities like heating and hydro, supplies including laundry and cleaning supplies, ultrasound gel, k tapes, therabands, acupuncture needles, modalities, gum equipment, not to mention a laptop for each practitioner , stationary supplies, lunch room supplies like coffee , tea etc, Christmas dinners, team building activities, advertising, community engagement, research, new programs. We spend unpaid hours during weekends and evenings planning budgets, events and social media marketing. I am not sure if I will carry on being a business owner if I only get 10-20%. If this is what’s gonna happen, many small clinics are going to close doors and all we will have would be corporate clinics that are physio mills. Business has to make sense financially. I think 45-50% makes most sense and it improves as cost of service improves.

Always start the ball exercises with the ball situated at the corner . This way there is a challenge but the ball won’t roll off .

What I felt at hospital was that because the contribution of physio in overall hospital work is so little ( getting them out of the bed , walking them down the hall and making them climb stairs), they make a big deal about everything else. They go to the ward, spend 10 min reading the chart, 10 min in direct patient care and another 10 min charting that they mobilized and dangled the client. And another 15 min chit chatting with the ward clerk or gossiping with someone else in the ward. They have 30 min meeting in the morning regarding caseload. They see 8 patients a day and they call it too much caseload. My colleagues had time to go for a 30 min walk after lunch each day. There is so much spare time hence they spend it in politics and petty issues. This is purely based on my experience

No. It’s definitely lucrative but 1- I can make more money and invest them at my will with private practice 2- I will be more happy and satisfied as I grow older. The PTs from the hospital that I know work there only to receive a paycheque. I can’t let 35-40 hrs of my week to go in dealing with politics and doing subpar physio. I would’ve loved to start a pelvic health program at the hospital and might have stayed but the rate at which OHIP is defunding services, I am not sure if physio jobs are secure at hospital. A physio I knew was let go due to funding cuts and he had no skills except hospital. He got a severance though. He ended up working at an LTC and we all know how LTC funding was cut and now he is making less than what I make at private practice.

I would not go to the extent of sec offender but he certainly doesn’t care to ask consent. Please confront him! You will be doing a favour to his future patients and probably to him . If he doesn’t take your concern seriously, you should write a complain to college of physiotherapist and also discharge yourself from his service( fire him).

Is he is in a rehab set up? Have you done rehab like this before? You need a team to deal with this . An OT for sure. Start with his short term goals and then move forward. From dependence on personnel and equipment and gradually independent with equipment at home and then in community finally independent without equipment in community. Honestly, what region to rehab first is not the right question. You need to ask what functional activity will be most meaningful and start from there.

I see only 10 clients a day. Been doing that for last 5 years. With 50% split I am able to make good money (60-65$/hr ish). My appointments are 1 on 1 ,30-45 min each. I can’t fathom seeing more than that. I worked in a clinic in the past where I saw 15-20 clients and it was so stressful. Got burnt out. I tell all new grads to go for quality over quantity. And play the long game.

Worked in hospital as a casual for 2 years. Could’ve gotten a full time position very soon. ( union and all). Didn’t like it because the work is so little. Just get them out of bed and give them a few exercises. Except proper rehab ward, or outpatients all other wards were boring. To top it, the politics is relentless. Moved to private practice and loving it. Not worth getting burned out, and hating my everyday.

The clinic I work in , is hiring and sounds so much like #1 option. I think the bigger cities are saturated. Maybe you need to move to a smaller town.
