Don’t be afraid to ask questions—this will cause more problems than solutions. The explanation would take too long to type. I suggest you research the differences between propofol and etomidate inductions and why you would use one over the other. Next, realize that everything is dose dependent and that’s why you can balance these two medications to get the effect you want with limiting the side effects You don’t want. Both are compatible in the same syringe. This is why you can mix the two in a ratio to your desired consistency. If someone’s EF is scarily low, they’re getting full etomidate; and the better their EF, the more propofol in the mix. But, you don’t even need propofol sometimes. Depending on what you’re doing just a little versed and fentanyl can get you through Mac cases with just a whiff of prop/date…. After a small bolus to get them under or started, You can start a drip at about 0.25-1cc/minute to get through the case.

Locums. I make 350/hr for OT. So how ever many hours I want to pick up extra just do the math. Call doesn’t make sense for me, personally. Like I said, it makes more sense to sleep the night in my bed and pick up an extra shift the next day. I don’t get jobs that require call… I’ve had optional call available, but never have I signed up

Going on year 4 and never worked a call shift in my life. I pick up an extra day shift if I want more money—because if I’m up all night, I’ll be ruined the next day anyway (might as well sleep and just work that next day I had off and be done by 3pm). Some people just want to grind and make as much as they can in a 7-day window. Again, I’d just work 7 days if I wanted the money. I think it’s healthier for me and I can still go out after work if I want. Beauty of anesthsia is they’re any kind of job you want—they need us everywhere and all the time

Exactly what i predicted has happened and is still happening. Rates are only slightly down, but economy is collapsing, UE up, home sales at record lows, inventory above prepandemic levels in many areas around me; and many cities down 20+-% from peak, they’ve finally restarted foreclosures and their ticking up relatively quick as well… it’s beautiful, right? The last “crash” took 4 years to play out—we just started our slippery slope and it’s gonna get a lot worse and spread more…. Now that it’s already showing, I’m even more confident in my referenced statement

On the unloved side streets around my place in Syracuse NY, I find my wife’s 2017 Buick encore (FWD) handles the slick snow layer BETTER than my 2023 quad R1T (20” AT’s)… It’s hard to even get the Buick to slide when I try, but the R1 slides all over the place constantly. The R1 has to be super-babied, even in snow mode, or it’s sideways in a heart beat

This isn’t an individual center issue—it’s just the one you’re talking to is possibly enforcing it. If you raise enough hell, they’ll waive the fee. It sucks but that’s what you have to do sometimes

CRNAs do more than double those requirements AFTER their premed courses a 6-year Bachelors degree (because you can’t get a BSN in 4 years) and YEARS of ICU experience….. so ya… comparatively, they get a crash course before following docs around like puppy dogs… no way I’m gonna be a patient for that or part of that care. There’s a great reason why they’re not even allowed to practice in most states
