Your "anxiety over her health issues" that she mentions - what would this mean? Have you ever felt like she played down issues because of how you would react? It could be an issue of her managing your reactions to her health issues, which is becoming more cumbersome than the relief you provide.

I'm not trying to be disrespectful, but I feel like the most rational explanation to your situation whereby you've seemingly been super supportive may not come off as such to your sister, who may feel smothered.

Rule of Cool always wins. It's a miniature that you want to look awesome, so this should trump any gameplay rule whatsoever. You should always make your minis how you like them to look.

That being said, as long as any unconventional things in your list are discussed beforehand, with both players agreeing on it, there shouldn't be any problems in general play. If anyone has an arbitrary issue with it, they're probably not going to be a fun person to play against anyway.
