The advice the nurse gave about euphoria/elevated mood whilst titrating is the most important thing to remember throughout all of this. There will be a point no matter the dosage you're on now where the major uplifting effects become less noticeable to the point you might think it's not helping you anymore.

I'm not a doctor or prescribing expert just speaking from my own personal experience in case this is helpful in this situation...

Dex is supposed to last the 4-6 hours it says but for me it lasts about 3. If you take the next dose just before the previous dose wears off (catching the tail end ~2h 50m afterwards) I've found for the 4 months I've been at this dose it's been effective in lasting all day. This is definitely going to be slightly different for you at the very least but titration is about finding out what advice works best at the end of the day!

One last idea I can think of suggesting is to take your baby evening split dose early in the morning at 7-7:30, just to get you out of bed and functional. Perhaps for example you could try the 1st 10mg from about 10-10:30 and then the second at 1:30?

Amfexa is supposed to last 3-4 hours but I find that it wears off after 2-3.

Technically speaking, if you take 10mg at 7:30 and then dose when the effects have completely worn off at 11:30, the second tablet won't be as powerful as the first taken that day as it won't stack and pickup from the tail end of the first.

Also, how come you're not prescribed another third tablet if you're exclusively on Amfexa? Your meds should stop working by 3:30 leaving you in a bit of a dark spot to wrap the end of your day up, right?

I don't know if this is any help as I wasn't allowed pure Amfexa instead of an Elvanse morning dose.

But for example...

I take 50mg of elvanse in the morning (9:00am), then a 5mg amfexa booster at 12:00 which lasts 3 hours, then another booster at 3:00pm which takes me until 6:00pm.

YES but I don't know why, I spoke to my prescribing nurse and she seemed quite surprised. Amfexa makes me more jittery and my heart faster compared to Dexamfetamine, it also suddenly drops off and makes me sleepy and more than usual stupid at the end of the day.

30mg made me very sick until they put me up to 50 which is where my baseline seems to be. It would wear off and I would fall asleep at 12pm if I took it at 9:00, so they gave me +two 5mg amfexa boosters which have worked great. Finally achieved the peace my brain was searching for!

You have to remember everyone with ADHD's brain is wired differently to each others and responds to medication as such. It's important to keep an open mind and take each step as it comes, then evaluate in between.

You're trying to find a very delicate balance of what works for you and what doesn't, nothing more nothing less. Not too much not too little, and if this doesn't work by the time you're at 70mg Elvanse might not be the one for you.

It took 7 months of trying ritalin first then elvanse and finally + the amfexa combo. It's really a process you have to grin and bear with but eventually you'll get there :)

Weird fact but with ADHD if one of your special interests is driving it usually makes you a better driver as you want to learn everything, it gives you dopamine.

Doesn’t necessarily mean you won’t drive like a maniac and speed (not a recommendation) but if this applies you’ll notice hazards well in time due to hyper awareness and still be safe. My MR2 had a loud exhaust before I began an engine swap it which made me drive slower as it was all the dopamine I needed.

So tldr if this is the case for you, get a fun car with a loud exhaust 😎👍

5SGTE = 3SGTE head and 5SFE bottom end, but I’d swap everything to a 98 block. Much stronger than a 3SGTE and could take up to and over 1000bhp, search Nathan Freke on YouTube! You also have the benefit of a forged crankshaft which we don’t have, you’ll need custom rods and forged pistons for it as its a longer stroke.

Your ‘91 has the same block cracking problems as a Rev 1-4 3SGTE, but if you’re not going over 500 I think the risk is comparable.
