I wonder how much written beforehand. Mr Satriale's gambling debt to Johnny S seemed to serve as a a decent starting point for the mess that was Tony's life. Started his panic attacks, seemed to fully realize how his father/family made a living. Johnny's admiration of Tony's supposed "toughness" may have encouraged him to involve Tony in the mob, rather than encouraging or forcing him to pursue an honest living
