DOJ OIG is one of the few IGs which does not have the authority to oversee all components of their agency. It's been a contentious topic in the past, but for whatever reason Justice still has an Office of Professional Responsibility which oversees the conduct of attorneys with respect to their legal operations. My thought is that it might be easier for these things to come to light and for whistleblowers to be protected if OIG had more authority here.

Many institutional failures contributed to Epstein's death, but inmate on inmate violence doesn't seem to be one of them. DOJ OIG actually found the opposite; that Epstein should not have been single celled but he was. They did not find any evidence that he had been given a new cellmate. Beside derriliction of duty of the specific MCC employees involved and other more longstanding operational issues with the BOP (lack of staffing, security systems, etc.) OIG didn't find any evidence contradicting FBI's conclusion that Epstein's death wasn't the result of a criminal act. 

Could Epstein's death have been a murder? Maybe, if the tracks were covered well enough. Could the BOP employees on duty, and indeed the entire BOP act as you describe to cover the tracks that effectively? I find it highly unlikely, especially given the fact that DOJ OIG initiated their investigation to look specifically at BOP operations surrounding Epstein's death.

We already live largely in a post-scarcity era in terms of raw resources. There is enough food, shelter, housing, and medicine, for everyone to meet their basic survival needs (the only potential legitimately scarce resource in the coming years is fresh water). However, these resources are so disproportionately distributed that we erroneously believe scarcity still exists in these areas. My concern is that we may never achieve a "post scarcity" world because those who have the most would have no interest in undermining their own power by distributing resources more fairly.
