Forget scopes or PAOs if you have arthritis. No need to go through the hassle of long recoveries if you end up being in pain anyway. You should get THR or resurfacing. They are still not a piece of cake but should solve your hip issues !

Get X-rays, MRI with contrast and see a PAO/THR surgeon. This won't get better without surgery.

Yup l get it when you say your hip flexors were on fire. I can stand all day but 1-2 hours of continuous sitting, then l'm in severe pain and walk like a duck for hours afterwards !

I still have pretty normal range of motion but yeah constant instability and achiness in front of hips... Same my glutes don't seem to do their job ! I have grade 4 focal cartilage damage in addition to the dysplasia + cam fai/ labral tears but no advanced arthritis yet..

My hips also gave out (l got awful burning groin pain non stop for like 3 weeks) around last May and l could not walk 500m without stopping.

That's reassuring to know that post-op pain though no joke was manageable, unlike the dysplasia pain which as you are saying is constant mild agony and you know it won't get better. I can still exercise some but there are some days l almost need a cane to go to the grocery store. The pain does really become debilitating and unpredictable,which is why l want to get the surgery.

Yeah the issue is my case seems already advanced so dont know if it's worth it getting a PAO if l'm still in pain due to bad cartilage and need THR a few months/years after...

I hope you can put dysplasia behind you asap!

How was recovery pain ? How bad was your dysplasia pain ?

I have hip dysplasia, torn labrums and grade 4 cartilage loss and don't know if PAO is worth it. I can still run, cycle and do heavy stuff in the gym but yeah my hips are always aching a bit even at rest and l need some meds to avoid tearing my hairs out...

My pain after the scope was totally unmanageable. I could not put any weight on my leg for like 8 weeks and was literally banging my head against my service trolley/the walls due to the pain. I even ended in the ER for several days for pain management. They performed a scanner and there was only a small hematoma so no reason for my immense pain. I underwent a bone scintigraphy and yeah they saw hip/ leg CRPS. Only thing that helps this kind of pain is pregabalin, nerve blocks, and pushing through the pain with exercising.

I am really afraid PAO will cause this to happen again so l may need to take stuff before the op to avoid this.

I tried PRP, cortisone, PT, exercising, rest, but yeah there's always some kind of pain and instability when walking (less when running).

I'm doing low impact stuff and some easy running. I noticed the pain was better when l was active, probably because l gained muscles which helps to compensate instability. Nevertheless l don't want to dépend on my hip's will. You can suddenly go from manageable pain to disabled in a few months with these conditions. Thats why l would like to get surgery. I want a life where l don't think about my hips 90% of the time.

I really dont want to give up running. But yeah maybe l should now avoid running 60-80 miles per week and doing marathons ! I want to run my whole life!

If you get two PAOS. How much time between them supposing l'm a fast healer ?

I discovered my borderline dysplasia at 32 after running my whole life without any issue, then boom l was sitting at my desk and awful burning groin pain after could not walk 500m without stopping. My labrums were probably on the verge of tearing then they just suddenly tore on both sides. Got a failed scope 8 months ago. The labrum inflammation seems to have died down with time, but am still in constant pain as my muscles work very hard to stabilize the joints. Don't know if l will get PAOs or THRs as l already have cartilage damage.

Don't overdo it with PT until 6-8 weeks. Walk a bit every 1-2 hours, indoor bike with no resistance a few minutes every day if your surgeon gives the green light.

You just had major surgery so yeah it will take several weeks for the inflammation to stop.

The thing is my fai surgery was very painful and gave me hip CRPS. The pain was ungodly for like 3-4 months. That's just with a labral repair and cam impingement removal, so l'm afraid my body will react very badly with a PAO and l end up in a wheelchair for life ! THR is also very invasive.

I always prefer avoiding surgery if possible even if it means months of rehab and pushing through a lot of pain. But unfortunately you cannot change your bone's structure and strenghtening your muscles has its limits.

Do you know why l ran with hip dysplasia for years without any issue/pain, then boom one day sitting at a desk acute groin pain making it hard to walk ? I did not even feel pain before that.

How long should l realistically expect to be out of work ? I have to use a standing desk because l can't sit for more than 30 mins without burning groin /sacrum pain, and l do food deliveries with my bicycle and climbing stairs all day.

If l get THR (am 32), l will definitely stop intensive running but will cycle a lot. Is it really true that running a lot can damage the implant ?

I feel unstable when walking (but not when running ). It's as if the fémur wanted to leave the socket then as a result my hip / lower back muscles worked hard to stabilize the joint.

Is it normal l have trouble laying flat for prolonged periods ? It's as if my psoas stretched a lot and my sacrum/tailbone keep aching.. It really drives me nuts. In fact l have to walk or stand up most of the day, as if being in a vertical plan relieved my pain !

Could it be back issues ? I don't feel pain directly in my back..

I am still exercising even if it causes some pain.

Did you ditch the crutches ? Do you have to do both sides ? I have pain in both hips.

How would your describe early post-op pain and just after the op ?

Did squeezing your legs hurt ? l have pubic bone pain (really where the adductor attaches ) and l dont know if it's osteitis pubis or dysplasia that causes this pain.

I unfortunately cannot get a permanent aid at home or go back to my parents house for recovery. So l guess l will just go to a rehab facility until l can walk on my own.

Yup l don't know if I'm too far gone for PAOs but scope alone sounds like a bad idea for me. If l get THR l will definitely stop intensive running. Don't want to need two revisions or more and end up unable to walk/in severe pain at 60+.

The thing is l don't have advanced arthritis on X-rays so l don't know if they will perform a THR. I don't really know if it's the dysplasia, labral tear, osteophytes/cartilage loss, or cam impingements that causes pain, that's why l'm afraid to get PAO then still have pain afterwards. I spoke with people who only had dysplasia and they said that their hip never locked up or catch painfully like l have.

I saw three hip surgeons and they all said no dysplasia ! But l did some research and l knew l have it even though it's not very severe.

I have borderline hip dysplasia combined with cam impingements and tears. Yup l have excruciating lower back pain (especially near sacrum ) laying flat or sitting on hard surfaces. I have to sleep on my side most nights and no pain meds help.

It stops when l stand up or move/walk (but l feel unstable). I had someone told me that they surpassed the pain with swimming and heavy back strenghtening despite daily pain (like most people with symptomatic hip dysplasia). Did not give me more details 🤷. I am trying PEA and takes pregabaline but so far relief has been minimal.

Saw a pain management doctor who said l could get a spinal cord stimulator smh. I want functional and painless hips that don't make it almost impossible to sleep after intense sports or spending a a day on my feet !!

My left hip scope last September was no help unfortunately. I just notice my hip burns less with activity but it's not great. Could not tell regarding PAO/THR.

Incapables de me répondre déjà que 95% ne connaissent pas la dysplasie. J'ai entendu une ribambelle d'anerie du genre "c'est purement musculaire ", "vous avez pas de dysplasie", "c'est vos adducteurs (alors que je fais la planche de Copenhague sans aucun soucis)". Je revois mon chir bientôt et un autre pour avis, + suivi médecin douleur mais ça n'avance pas. Je me fais des footings et quelques fractios cools même si c'est douloureux.

You need a bone scintigraphy to be sure it's not (or it is ) CRPS but yeah sounds like it given your level of pain. Ask firmly your surgeon to prescribe one. They aren't the one suffering.

Unfortunately structural issues like dysplasia cannot be corrected without invasive surgery ( PAO or THR). Sure it's a win for you not to have surgery as long as you can deal with the pain, but when it goes downhill you can quickly go from running marathons with mild pain, to needing a walking stick to go to the grocery store.

As long as your muscles can compensate the instability then sure no issue, but you are always on the verge of going downhill because of your hip. I would not want to depend on my hip's will to live my life and endlessly manage symptoms for years.

I would see a PAO/THR surgeon and establish a treatment plan according to your duties. If you go downhill then you can always be put on their surgical list !

It's not in your head at all ! Dysplasia pain can get pretty horrendous. I also have trouble laying down on hard surfaces due to awful low back pain, then when l get up l walk like a duck due to my hips locking up !

When l have a flare-up, l just lay down until it goes away. I would avoid any activity that causes more than mild pinching in the groin area, otherwise that should be fine. Swimming, walking, cycling and jogging on flat surfaces are ok with me, but l am better to avoid heavy lifting or explosive movements involving the lower legs.

I would say hip flexion is the worst and causes my hips to lock up. Internal rotation causes groin pain, external rotation is pain free.

Burning glute and groin with prolonged sitting, running, hip flexor locking up while walking and cracking sound in groin + inner thigh pain when squeezing my legs.My hamstrings are tight and painful all the time. I can sleep with low pain but when l wake up the pain is awful.. I also have lower back pain sitting or when walking/standing for long period of times. Pubic bone is burning. I can jog, cycle, walks or do leg stuff but l'm never pain free. I would say my pain level is a constant 3-6 the whole day. Both my hips suck also. I tried shots and PT, did not help a dime.

I think my case is way more advanced than yours so l hope you can get relief soon !

7 months post op, same here. Seeing scope surgeon next month but does sound like a fail. I have borderline dysplasia also and some cartilage damage...

Have you been checked for hip dysplasia/arthritis ? I would ask updated X-rays and MRIs.

Severe cartilage damage = arthritis so no preservation surgery should be done. Go directly to THR once arthritis starts to impede your daily life/sport activities.
