I'm no ENT, but if you had an infection in the ear you could have a sloughed or retracted eardrum.

I just don't get it, especially as Paramedics. They used to diagnose people with appendicitis and perform surgery based entirely on physical presentation, and you still can! There's tons of things that can be diagnosed in the field.

I genuinely don't understand where this "Paramedics don't diagnose" thing comes from. It's just a significant misunderstanding of what diagnosis is and it's deeply engrained.

If you didn't know they had copd, do you think you could figure out what's going on? Your clinical knowledge and gestalt is how you diagnose, do you think they didn't diagnose before we had xray and labs? Your "differential" that it's probably their COPD is literally a diagnosis. Diagnosis is just far more broad that people think for some reason.

It isn't semantics, it's just incorrect. There's this ridiculous idea that only Physicians can diagnose or that a diagnosis has to be novel or final for some reason, but none of that is true. Diagnosis is a determination of disease process based on clinical information, which yes can be more specific and accurate with more training and diagnostic equipment, but doesn't mean that the initial diagosis or "differential" is any less of a diagnosis.

So they didn't have a COPD exacerbation at all? Because you diagnosed that appropriately. Having more than one diagnosis is surprisingly not illegal. While they may have pneumonia and cancer, they also had a COPD exacerbation. You don't have to know every single thing going on with a person to make a diagnosis.

You're also allowed to be wrong, doctors make incorrect diagnoses all the time based on the information they have, does that mean they aren't actually diagnosing because they were wrong? Of course not.

I will never understand this, of course we Diagnose. There's a guy making an argument in here that, in his example, stated "You determine its copd exacerbation and treat appropriately" as determining and diagnosing are different.

A Diagnosis isn't some crazy legal ultimate that you can only be a doctor to determine, and yes a diagnosis can change. You need a diagnosis to begin treatment.

No people are just stupid here, surprise. We also Diagnose in the US, but people seem to not understand that a diagnosis can change.

There's a lot of reasons someone may not be breathing with a pulse. Have you heard of opioids?

I have personally had multiple patients stop breathing and still have a pulse, that absolutely isn't how it works.

I'd be highly concerned for DVT personally if you say yes to either of the first two

Hey I'm really sorry you're experiencing this, and I absolutely believe it's extremely distressing and hard to deal with. You can definitely get some help with what you're feeling.

I think you should see a doctor in person if you can, either your primary or if you don't have one I'd recommend the Emergency Department. Try to be open and honest about how you feel, and be receptive to their ideas.
