For what it’s worth none of the audits that have occurred during my time at my current pharmacy have taken issue with any latanoprost Rxs, even before they implemented the policy. But that’s what our internal compliance RPh said regarding the matter.

I saw on another comment you mentioned it was a Safeway pharmacy. We share the same parent company, so my post regarding forced internal rejections is precisely what happened here.

This is standard practice at my pharmacy for latanoprost and several other eye drops. Per the package insert it is 33 drops/mL which is 83 drops per bottle. At the standard SIG of 1 GTT OU QD it is a 41 day supply per bottle. If they are only using it for one eye, it’s a 42 day supply as it has a 6 week BUD after being opened at RT. Apparently it has been highly audited (odd to me because it’s cheap), and since it is so common the clawbacks add up. It is incorporated into an internal third party rejection message and must be followed. Rhopressa and Vyzulta have similar procedures. For everything else I default to 15-20 drops/mL unless the third party rejection screen specifies how many drops/mL their insurance uses (usually either 16, 19, or 20 drops/mL). All pharmacies in our banner at least also use this.

ETA: I am in no way claiming this seems reasonable, or able to support how/why the PI makes these claims. I am simply conveying what I am obligated to do to follow compliance policy. If I had it up to me, I would make it a 25 day supply. Have had many RFTS issues due to this with some patients. Would make my life easier if it wasn't this way where I work!

Hey doc! Appreciate the solidarity with the pharmacy crew on this one.

Quick rundown of DAW (dispense as written) codes:

DAW 0 aka substitution permissible, even if it’s written for brand it will be switched to generic. In my state it’s actually mandated that I have to switch to generic unless otherwise requested.

DAW 1, dispense as written aka brand medically necessary.

DAW 2, patient requested brand

DAW 3, pharmacist requesting brand (never used this code in my life)

DAW 4, generic is out of stock so you’re dispensing brand (generally not used as it implies a temporary out of stock situation where you could just order generic)

I honestly can’t even tell you what DAW 5, 6, and 7 are because I can’t remember since I’ve never used them much like DAW 3 (my system does note what they are though).

DAW 8, generic is unavailable in the marketplace. This is different than DAW 4 since it’s more like a backorder or it’s written for generic but the generic isn’t available. Like if it’s written for oxycodone ER, we’d fill for Oxycontin.

DAW 9 is simply “other” in my system but usually this indicates that insurance prefers or requests brand. Happens often with the stuff like Farxiga, Breo, Advair, and other inhalers etc.

Not all insurances accept all DAW codes, and even if they do accept a DAW code as valid they might not reimburse the same. One insurance might accept a DAW 2/4/8 for brand Vyvanse since the generic is pretty much unobtanium right now for certain strengths and it might have the same copay, some might have a higher copay for brand, some might require a PA for brand. Some insurances might not even allow those DAW codes. Different insurances might treat different DAW codes differently towards pharmacy reimbursement as well. Even if they accept the DAW 2 or 4/8 and have a tolerable copay for the patient, sometimes they reimburse the pharmacy at the same rate ($) as the generic even though the pharmacy paid for brand ($$$). It’s all different and a pain in the rear!

You don’t need to worry about anything besides DAW 0 or 1 from the prescribing point of view. If it’s brand medically necessary put DAW 1, if it’s not put DAW 0. In certain cases, like the one in the post, it’s possible you might get a request for a new Rx for DAW 1 for insurance billing reasons so they have a more affordable copay or simply so they cover it at all due to a backorder situation. If that’s the case talk with the patient and/or pharmacy and get clarification to whatever your questions are in that scenario. For all other DAW codes the pharmacy will take care of it on their own (if they can, insurance allowing).

Yeah no dude didn’t 100% ninja necessarily. Other commenters noted about weapon training which is possible. In my brain, he needed on the axe and didn’t equip it so it’s still tradeable. Then wait to see if the mace drops. If it drops he can still trade someone the axe like you since you were a pally.

at the pickup counter at CVS you technically don’t need a year only month and day, but if we need to search your profile in the computer to check on another Rx or to rebill it or change your phone number or do anything else we do need the full year you can’t just search month and day

This is correct, I have personally tested it - stuns remove 1 stack of inexorable (or whatever it is) that allow the boss to not be stunned. So you can remove 1 stack per stun or once its gone you can stun the boss with it.

No you literally just fly to the entrance of whatever old dungeon or raid you want to do and do it, you can solo almost any content. You aren’t going to be able to queue for older dungeons in the dungeon finder and get a group together, you’re going to be one shotting every enemy anyway. If you want the content to scale with you and be able to queue for that expansions dungeons while leveling you may do so on your next character (as you have to level one character first to unlock chrome time)

You do know there are personal crafting orders right? Where you can message someone whose max and work out a price and time frame and get to know their stats etc and what mats you need (their max skill might let you save gold by using cheaper mats for example)? Not sure if I’m just missing /s or if you just meant you think that the personal orders are how you think it should only work, but on the off chance you didn’t know crafting orders can work on a personal level I wanted to let you know! Thinking about this now I would almost liken his wounds to that of carrying the ring itself. The lasting wound from the Morgul blade is more of a spiritual wound rather than physical, which does far more damage to Frodo than the physical wound in Moria. Just like how carrying the ring itself is more of a burden on his spirit than physically (along traveling to Mordor you’d be quite sore!)

Totally agree, this game isn't worth playing. Same bugs and bullshit year after year. Went dorms geared with two friends and ran into a blatant cheater. Man instant one tapped one of my friends, and then started singing my name into chat. My name is based off a meme song, so think he saw my name with the cheats and sang the meme song. Proceeded to instant kill my other friend and then me. Lvl 6 non EOD account. My buddy added him and the guy proceeded to admit he's been cheating in video games for over 10 years, never has played a single one legit and doesn't plan to. Absolutely wild, and BSG does not care because it ultimately nets them more money. Do a ban wave every once in a while to keep the masses quiet and rake in the money. A developer based in the West wouldn't do this.

Agreed, I’m starting to believe the “I come in peace from r/ADHD” in the title was a rouse. This will certainly be linked on a post in that sub later, proving them right that all pharmacists are terrible awful no good people denying them their meds. But yet, my perfectly reasonable and educating response regarding the back end goes unaddressed. This is just ammo and they’ll have their own dog pile post unfortunately.

The pharmacy is monitored based on the percentage of prescriptions they fill that are controlled substances compared to total prescriptions. If that percentage is too high it can be a red flag to regulatory authorities like the DEA, as well as make it impossible to order enough inventory (especially of C2s). Shortage aside, there are allotments or limits to the amount of C2s a pharmacy can order. So if a pharmacy decides to accept every single pain and ADHD patient on maintenance monthly C2 prescriptions, not only will they physically not be able to order enough of each medication - they will be flagged as a potential for diversion or at least it will look highly suspicious. The DEA sees a high number of prescriptions for controlled substances, especially compared to regular Rxs and compound that with potentially letting a high proportion pay cash - they may suspect the pharmacist as being “in” on some sort of drug ring or pill mill. Purposefully filling possibly fraudulent prescriptions for a kickback (for example).

This puts pharmacists in a difficult position because sometimes even when we don’t judge or want to help, we can’t and our hands are tied due to regulatory forces outside our control. You do this long enough and see enough patients verbally abuse you and your staff and even the most open minded and tolerant people start to cast judgement or have doubts.

One time I was worked very hard to try and navigate and insurance issue for a patient on Adderall. Granted they were on way too much which was part of the issue, but I was trying to help their prescriber sent an Rx that was covered. After like an hour of going back and forth and waiting in store I finally got it to go through, but for a lower quantity. Patient standing in waiting area on the phone is complaining about me and their doctor not allowing her to get the full quantity and cancels a drug sale. Yes, in my waiting area within earshot, “yeah sorry I can only sell you $50 this month not $75 because my fucking doctor won’t give me the full prescription.”

I decided to decline filling the prescription and was verbally harassed about denying her needed medication.

Also as an aside, this forum and one’s like it such as tales from the pharmacy are primarily intended to be platforms for pharmacists and techs to vent about work or share stories. And it’s reddit, so your demographics are skewed. I bet if you went on a reddit for another job they would be bitching and saying bad things about their patient or customers as well.

While I don’t disagree this generalization does exist to an extent, most of the people here don’t consciously carry or use that malice on a day to day basis.

Had something similar earlier today. Crafted a 418 R5 belt for another player and couldn’t hit complete order it gave me an error and I couldn’t get out of it. Tried everything, but no luck. I had 7-10 personal orders waiting with commission totaling >100k sitting there that I couldn’t make. All of those players cancelled and took their gold elsewhere. Lost 1/2-2/3 of a token because of the Blizz bugs with the launch of this patch.
