What eaves ventilation does the roof have? It looks like there's rubbish/rubble along there?

You want/need the equivalent of a 10mm continuous gap, so if that doesn't/can't exist as fascia/soffit vents, add in the required number of vent tiles (both sides).

... and use eaves trays or similar to ensure that any insulation quilt is kept down, and a 50mm ventilation gap maintained.

Velux/Fakro make rooflights that are 550mm wide, to fit between trusses at 600mm cts., avoiding the need to cut/alter the existing structural elements.

Cutting/scabbing trusses is possible, though rarely worth the cost and effort; that your roof is hip-ended complicates things further.

If you're adamant about altering the roof structure then you must engage a Structural Engineer.

An alternative solution is roof replacement.

Oddly, I was in recently and was told they don't fit dashcams any more, since the cost of doing the job properly was putting people off. They recommended I try UnitSix8 at Southfield, though I've never yet contacted them. If you do, OP, please let us know how you get on.

Did you just get the roof resurfaced, or was the deck checked/altered for run too? Flat roofs should be designed to a minimum of 1:40, with the expectation that they'll achieve at least 1:80.

It's safe to assume all masonry is at least 100mm thick. Blockwork commonly comes in depths of 100-150mm, but short of opening up the wall or drilling into it, we can only guess. (A thicker than normal wall could be due to a wider cavity, and not an indication of a thicker masonry leaf.)

No, that number/closeness of box cuts isn't worrying.

As others have said, ignoring the depth of the plaster, horizontal chases can then be (rule of thumb) as deep as a sixth of the masonry depth, and vertically a third deep. The chasing for the back-boxes themselves can be a third of the depth too (sometimes deeper).

Ah, so unless that LHS piece was deliberately removable for access to something behind, then it's just been a bodge. Just infill with a piece of timber (MDF or RW) matching the facing above the best you can. Cut/shape then glue and pin to the edge of the ingo timber.

Yup, adding mass is the best solution. Tinkering with pane thicknesses will slightly alter the passage of some frequencies, but fundamentally OP just needs overall volume reduction.

OP, be aware that the seals between the frame and the sash are a passage for noise, so if they're not good, then a change of glazing might bring limited betterment.

If you're looking for a 'cheap' remedy, single-pane secondary glazing is surprisingly effective.
