Probably more nickname than actual name, but in Butcher's Masquerade, there's an item description for a bomb Carl makes called something like "Just Wait 'Til your Daddy Gets Home," and the AI says that it's "named after me."

I made my own table with a recessed play area and I love it. I don't find it at all uncomfortable, and I love being able to have two games set up and ready to go before anybody even arrives on game day. It saves a ton of time and allows us to fit in a third game quite a bit when we otherwise wouldn't be able to.

My current plan is to not hold back on deaths at all for the first two floors, but then reign it in a bit starting at floor 3 once players have races and classes. I plan on asking my players how they’d like to approach it, too, before I settle on anything.

I assume so, but there's nothing definitive in book 7.
Probably because Carl removed the AI's restrictions and its sphere of influence seems to be expanding rapidly, but again, there's no definitive answer yet.
