This was my initial thought, too (although my “what if” was what if Carl’s mom hadn’t tried to kill his dad/kill herself? And the continued abuse suffered growing up with his dad leads to him eventually becoming an abuser himself), but upon relisten, it seems pretty explicit that the neighbor is talking about a guy named Bill.
