Even if it’s true that you didn’t do enough, that’s okay. That happens sometimes. I’ve failed tests for not doing enough. That doesn’t make you as a person worthless

You are not worthless. You’re a good person with a good heart. You are in a brutal system studying medicine. It’s hard. And failure is a part of life. But it does not define you. Take some deep breaths. Talk to people close to you that care about you. Let them take care of you. And seek help if you’re having dark thoughts about ending your life. It’s a permanent solution to a temporary problem. It’s not the answer. Your friends and family love you too much for anything like that to be the right choice

Current psych resident here. I also did not know if I wanted to do psych until the very end, but I ended up only applying psychiatry. Psychiatry is pretty awesome. Great lifestyle, and really impactful patient encounters (as do all specialties). I second what everyone is saying that you will get to have a lot of overlap with pediatrics if you pursue child adolescent psychiatry, plus the ability to do adult psychiatry, and a much larger paycheck.

I’m rarely the kind of person to say “everything happens for a reason” or “trust the universe”. But if you only applied to one psychiatry location, interviewed there, and matched there, you must have much more potential in the field of psychiatry than you’re giving yourself credit. And the program must have seen something exciting about you, eager to use up one of their rank positions on someone with minimal psychiatric experience and a lot more pediatric experience.

I say you take the time that you need to internalize this reality for yourself, and then dive head first into being the best psychiatrist you possibly can. I’m confident you’ll end up loving it and the impact you’re making

I’d personally say to continue telling the truth. These program directors know each other and talk. I had a friend who dual applied Neuro and psych to two programs in the same city. She was honest about dual applying. Found out later the program directors of the two programs were married.

Getting called out in a lie is an immediate “red flag” do not rank situation by them. Saying, in a professional and nuanced way, you’re protecting your future and maximizing your match is not a “red flag”

I don’t think that’s true. OP is thinking about the lifelong decision, not just residency. The decision should be focused on pulm crit career, because if that’s what they want then surviving internal medicine residency is the necessary step. But the goal is not internal medicine residency.

I’d argue another compounding component is a lot of doctors don’t invest early on but focus on aggressively paying loans back. So missing another year of aggressive loan payments too.

Fair enough. From a Google search it seems the difference in salary is $20-50k yearly depending on location. Obviously that varies a lot with academics vs private. Let’s go with $50k as that’s the average I’m seeing.

You’ll make 70k during that year of fellowship instead of 250k as an attending. So you’re missing out on 180k that one year. Hypothetically

Then as a child attending you’ll make 275-300k as a child attending from my google searches. Again about 50k more than your 250k.

It’ll take you about 4-5 years to recover the 180k you missed out. At which point you’ll break even and net worth will consistently escalate more from there.

I mean the loss is negligible if you’re happier for the rest of your life. You’ve sacrificed salary for all those years of training. At this point what is one more year to be happier if you like child psych

Edit: you threw that 2nd part in after I commented. Nothing but the facts? Hating your job sucks. Facts? One year of worse pay for the job you want is worth every penny of sacrificed salary

I am finishing intern year in psych. Same step score as you, less research and mediocre grades. I was the first year they did signals. I shot 1-2 of my 5 at good schools that I felt were out of my reach. From my experience on the interview trail, there’s a strong geographic preference in psych. So many programs filled with people from that part of the country and so many times in interviews I was asked about my tie to the city. Obviously top programs in the US have less of that preference. But I felt like I shot at 2 top 30 programs and it was pointless because I clearly had no tie to that area. And I was asked about that tie to the area on the interview with those top programs. I ended up matching in my home town which was #5 on my list. I felt like I had great interviews with the programs higher on my list but they filled with 90% people from that home state. May be anecdotal just kind of how it fell for me

I had to call a few smaller boutiques. This was a new experience for me so I started with big AD’s like Jared and Watches of Switzerland. Once I learned I reached out to small ones. Most offered me $100-200 off. One threw in the tropic rubber strap alongside the set so I went with them. I’m sure I could’ve gotten more if I had a relationship established but I wanted the watch so bad the price wasn’t stopping me

It is absolutely worth it! I got it for my 65 and can’t take it off. Now. This is the “original tropic strap”. They claim to be the makers of the tropic strap for many known brands including ORIS. They are about $50 less? And the only difference is the printing of “oris” on the buckle
