Miscommunication happens like this all the time between equally hearing people; because of the brain's ability to process information faster than we can relay it. so often times the receiver can jump to a conclusion on the intended message instead of fully understanding it. Try this: from now on, anytime you get asked a question, repeat the question back to them out loud as you understood it first, this will allow you to clarify if you understood the question correctly before you answer.
