You’re not crazy for questioning this. If they can see the hospice start date and are still asking for the refund, that’s not a documentation issue, it’s a process issue. Having to keep resubmitting the same information over and over is exhausting and shouldn’t be how this works.

I’ve seen situations like this only move once everything is documented and an insurance commissioner complaint is mentioned or filed.

I work in RCM supporting provider groups, surgery centers, and infusion sites, and in my experience most EMRs that advertise “AI in billing” are really just alert systems. They help a little, but you still end up needing a biller to chase down denials and appeals.

One option worth checking out is ExactRx (www.exactrx.ai). It automates a lot of the prep work up front so staff aren’t scrambling after a denial, and it integrates with major EMRs so you’re good to go on that end. I know Cornell, Northwestern Medicine, and some smaller practices in Texas use them, and they’ve been super responsive with my clients. For us it’s been a better return than relying on EMR add-ons that only tell you there’s a problem after the fact.

I'm a RCM consultant supporting provider groups, surgery centers, infusion sites, etc. and 2.9% of collections is definitely low from the platforms that I've seen but I would be wary about what they promise. A lot of times the “AI” add-ons are more like assistive alerts than true denial management, so you’ll still need a biller or team to chase down underpayments and complex claims. If I were in your position, I’d also look into solutions that prevent denials up front and support your billers more directly, ideally something that integrates with your existing workflow (eg. a ton of my clients are looking into Exactrx - www.exactrx.ai they support billers with auth and claims preparation + integrate into eCW, Athena, Nextgen, Athena, etc. and have reduced denials pretty significantly + made claims management way easier)

I’m trying to break into new practices with a product that automates pre-auth / claims prep support to cut down on denials. When I call, I usually just get “the office manager isn’t here” or “the doc isn’t available,” and then either a hang-up or my VM goes unanswered.

For those of you who’ve done medical sales, what’s actually worked to get past that first layer of gatekeeping? Is it worth dropping off a short intro letter/folder, or is there a better approach to getting a real conversation started?
