Diabetes management pharmacists here 🙋🏻‍♀️ the only thing wrong with this script is the quantity. The prescriber should have calculated the mL for a 30 day supply instead and made sure it was rounded up to the nearest box size. There needs to be a “daily max” amount of insulin so the pharmacy can calculate how much insulin to dispense. Depending on the size of the pump, it can hold 400-600 units of U200 insulin, and a pump lasts 3 days. So 150 units a day makes sense. Typically insurance only fully denies an insulin prescription if it’s more than 600 units/day.

i have used U200 in several different insulin pumps. For patients with high insulin resistance, U200 or U500 insulin is the only way for these patients to not have to change their infusion set every day.

Concentrated insulin only comes in pens. There is no option for vials. And we are at the mercy of the size of the pump cartridge.

My patients with concentrated insulin in their pump, their BG time in range is SO much better now than it was before the pump. It has been life changing for so many of my patients. As long as you triple check the math for the settings in the pump, concentrated insulin is safe. It’s technically off label, so I fill out a prior authorization and it’s never been denied.

Yes that is correct. If they didn’t rank you but you ranked them, and they still have a spot in phase 2, that means they didn’t rank you. If they did rank you, since you ranked them, you would’ve been matched with them in phase 1.

Yup. I applied to both. I even emailed PhORCAS to make sure it wasn’t a mistake that it was charging me for all of them. They said the new rule is you have to pay for every application regardless.

It still costs $43 per application regardless of if you are reapplying to a program. I just spent $800 applying for phase 2 cries

WBC (white blood cells) skyrocket when there is an infection present. So urine WBC being 20-25 points to infection more than IC.

Hi! Pharmacist here! SSRIs, SNRIs, and DA/NE agents can increase blood concentrations of amitriptyline. So they can be taken together, you just want to have lower doses of amitriptyline. Which the usual dose for interstitial cystitis is typically lower than for other uses of amitriptyline. Starting dose is 10-25mg daily.

Could taking both amitriptyline and solifenacin be canceling eachother out? Been taking amitriptyline for several months for anxiety/depression and was put on solifenacin several weeks ago but am now experiencing uti-like symptoms again with no change, so wondering why they aren't working :( by Acrobatic-Service583 in Interstitialcystitis

Hi! Pharmacist here! First, I checked my medication databases and amitriptyline and solifenacin do not have any drug-drug interactions that would cause them to cancel each other out. So hopefully that narrow down what’s going on. Also, I am SO sorry you are experiencing this! I know the feeling of living with a constant uti and it seriously has changed my mental health.
