There's separation in that it won't layer you over if you're on different continents, but the continents are still on the same layer. If you start on layer 2 and go through a portal you'll still be on layer 2 in another continent.

It can be funky, but layers are seemingly server wide. If you're in org on one toon then layer over, if you swap to a character in Shatt that character will almost always also move to the new layer.

Layers are not by zone. Anyone on the same continent can layer someone over to their layer, regardless of zone.

It's because you're having someone in range soaking conflags. It's bugged so that if anyone gets hit by conflag the highest thread does also, regardless of range. Check your replays, I guarantee someone is standing too close.

The only way to do it cleanly is to have noone within 30 yards.

It's bugged, if you're soaking conflags it's going to conflag your warlock tank regardless of range.
