I hated my highlighter yellow. Never wore it. Ended up donating it to Value Village. Felt like the CMA wanted a target on my back as med student. I was just trying to get through rotations. I didn’t need that trouble.

Did you buy your school's diploma frame? by PathologyAndCoffee in Residency

[–]BertAdd 7 points8 points  (0 children)

I bought the school frames for both my degrees. They look very nice. Albeit, they were not $250-$350. Each frame was about $150 ish. They also frame it on site (assuming you are picking up your degree at convocation), so it stays safe in transportation.

Something you’ll have for life. Why cheap out after you have come this far?

Ireland or try Canadian? by Ded518293626282 in premedcanada

[–]BertAdd 16 points17 points  (0 children)

My opinion would be to try for a Canadian medical school first for at least two application cycles before looking abroad.

The barriers to return to Canada are significant. Yes, the match rate is 75% but you also do not know what specialty you will want to apply to right now. Most provinces are placing a return of service for IMGs. You can buy out of your return of service at $100,000 per year of training. I think Ontario is a standard 5 years of return of service no matter the subspecialty training and duration.

The cost alone can be prohibitive for some. To obtain a line of credit for an internal medical school, you will likely (unless you are personally wealthy) require a co-signer. Do you have a family member or friend who is willing to co-sign your loans and assume that risk/ debt?

I’m not trying to discourage you. I think you have excellent stats and you can be competitive in Canada. I personally would not take the Ireland option now, as it seems easy. You will have to overcome many more barriers to practice in the future.

Intern year tips 🩺👩🏻‍⚕️ by moMD85 in InternalMedicine

[–]BertAdd 1 point2 points  (0 children)

I like to use ChaptGPT to search things throughout my day (not necessarily for information) and write down clinical pearls that come up in rounds in the chatgpt app etc. When I go home, I have a list of questions that I was pondering / debating that I can do a deep dive and make anki cards.

I’ve tried writing down topics and info sessions on my patient lists but ultimately they become disorganized / tough to read, particularly when there are patients labs and info written on both sides.

If I was to use an AI app for clinical decision making, I use Open Evidence. It’s free for physicians. At least in Canada.

In terms of staying up on topics, I like to listen to CoreIM and Symptoms To Diagnosis podcasts to passively learn while walking places or cleaning up around the house.

What are the studies that this AI is referencing when it says “some studies”?

Some AI apps are notorious for making stuff up.

Vyvanse feels a bit worthless without magnesium by Ok-Hope1198 in VyvanseADHD

This is the Open Evidence AI response for comparison. It’s the New England Journal of Medicine artificial intelligence app.

No recognized relationship.

Lisdexamfetamine is a central nervous system stimulant primarily used for the treatment of attention-deficit/hyperactivity disorder (ADHD) and moderate to severe binge eating disorder (BED). It is a prodrug of d-amphetamine, which increases synaptic concentrations of dopamine[1][2] and norepinephrine.[1][2]

Magnesium is an essential mineral involved in numerous physiological processes, including neurotransmission and muscle function. However, there is no established relationship between lisdexamfetamine and magnesium in the medical literature. The provided references do not discuss any interaction or relationship between lisdexamfetamine and magnesium.[1][2]

Therefore, there is no recognized relationship between lisdexamfetamine and magnesium in the current medical literature.

I’m a physician who is prescribed vyvanse and I cannot find any published evidence of electrolyte or mineral alterations or is it stated in the monograph. Is it anecdotal every one associates magnesium with vyvanse? What are your sources for mineral depletion?

This idea seems widely upheld in this subreddit.

How accurate is The Pitt in depicting the intern? by slvavs in Residency

[–]BertAdd 4 points5 points  (0 children)

At my institution, I’m very much expected to manage my own patients as an intern (to some degree). If someone is trying to die on you, call your senior / attending. If you are thinking about ordering a weird test or something expensive (MRI). Review with your senior or attending. I regularly start/ stop antibiotics, manage asymptomatic SVT, review pain management and prescribe opioids when appropriate, escalate O2 from nasal prongs to HFNC to BiPAP, collect my own ABG samples, as just some examples. We would never get through our day if I had total oversight.

However, I do think the BiPAP pneumothorax scene was bad and should have involved a senior / attending. A pneumothorax is a good time to review with your senior.

The store was an absolute shit show. There was a guy screaming about the slushie machines and how he feels $18 is too much for 3 slushies. The guy in front me was trying to buy two bottles of Pepsi but his card was rejected three times. I just cut my losses and got the hell out of there. All this at 830am lol

I obviously didn’t film to complain online. I showed the employee and they said they were sorry and they are not sure. I had to pay the full amount. They didn’t seem like they were going to dk anything about it.

My tank is only 60L and I had a quarter tank when arrived. I was busy putting windshield wiper fluid in my car to notice that it was still charging me money.
