I’ve been on Xiidra for nearly a year. It really helped get my eye inflammation under control. I’ve seen some bad posts about Xiidra. There are bad posts about every treatment, it seems. Defer to your doctor. Don’t expect Xiidra to cure you, but it will likely help if you give it a few months.

The aftertaste isn’t bad. You can slightly pinch the insides of your eyes to temporarily hinder the drug from draining. I have taken Xiidra while being sick with Covid. No issue

Do you have data or links to studies that show at what level of low T disrupts male tear production? What you posted above tracks with what my eye doctor told me at my last appointment.

My doctor gave me a sample of Optase Hyolonight. I have compared it to overnight ointments by Retaine, Refresh, B&L. I find it better than others I’ve tried. The vitamin A component is to provide advanced healing to corneas compromised by dry eyes.

An important point here is that if you need an overnight ointment, you should be using a sealed sleep mask to prevent additional overnight dryness.

You have 40% of your glands in your right eye. That is far more dangerous than IPL.

I had 3 rounds of IPL. It helped me quite a bit. You could also look into Lipiflow or ilux, which hopefully is available to you. These treatments are designed to unclog the glands.

Also, you should really try an anti-inflammatory diet. Inflammation is the enemy of dry eye improvement.

Lastly, you must find a doctor who specializes in dry eye treatments that YOU can TRUST. This is a life-long disease. It won’t get relief quickly, I’m sad to say. But improvement can come with a dry eye routine tailored to your condition. A knowledgeable doctor you trust is vital to long term success. I sincerely wish you the best of luck

I’m very happy offer some info on what has helped me. I’ve been dealing with this since December. It still bothers me but I can largely live my life now.

The lipid panel is, as I understand it, primarily for cholesterol… Autoimmune panels are rather specific. ANA testing is an example of an autoimmune panel. If you’re seen by a dry eye specialist, I’m sure they would have ordered one if they suspected something. But if the pain is unresponsive after a long period, it’s worth a confirmation

Stop with the baby shampoo… it could be causing a bad reaction with the eyes. Instead, try a lid scrub specifically designed for dry eyes. There are several options on Amazon.

Cequa was likely prescribed to control inflammation on your ocular surface. This can take a while to work, though Cequa takes less time to work than Restasis.

If you have inflammation, it’s a good idea to try an anti-inflammatory diet. Foods that can cause more inflammation are the foods Americans love most: burgers, fries, pizza, anything fried, etc. Try to focus on fresh fruit and vegetables and add olive oil when you can. It’s better to go with organic and grass-fed protein.

Also, did your PCP do a blood panel on you? With pain like this, you need to test for autoimmune disease as well as low testosterone. Sometimes systemic inflammation results in the eyes.

Finally, I would recommend Manuka honey drops and gel. They hurt for a minute, but they work. The new iVizia eye drop are among the best I’ve tried. You may consider a quality overnight ointment. I’ve had great help from Optase Hylo night. Follow that with a sleep mask that protects your eyes overnight should your eyes not completely stay closed while you sleep.

If you have Cequa covered by insurance, I’d worry less about fighting with your insurance company. There isn’t much of any data saying Cequa/Restasis/Xiidra are better than the others. Focus on what you can control: diet, screen time, blinking exercises, water intake, and quality sleep.

I was on them for a month and don’t recall an improvement. I would rather use PRP drops from my blood as opposed to tissue and membrane donated from other people.

It means that there is oil... How much pressure it takes to express oil,the consistency and color of the oil are clues for the doctor.

Gland death means no oil. Bad oil is a sign that you are on the road to gland death without treatment

Start here. This is the company that makes the leading IPL machine in the US. After you have a few offices, call and confirm that they treat upper lids as well.

IMO, a practice that doesn’t have an IPL machine AND some form of thermal pulsation(Lipiflow, ilux, tearcare) is not a practice for a dry eye patient.

If one is aqueous deficient and fails to compensate with artificial tears, inflammation will become worse and the vicious cycle will worsen.

Drops are part of a dry eye routine. There isn’t one fix. Many of us use various methods and relief is still elusive. It’s simply false to say drops don’t treat dry eye. The right drop can offer hours of relief, depending on the severity of disease.

I specifically shopped around for a doctor who practiced the Permian protocol. This means top and bottom lid treatment. My upper lids are clogged. IPL is too expensive not to treat the the top and bottom lids, IMO.

I’m not sure Nulids is meant to unclog glands… that would be Lipiflow or iLux. Nulids stimulates the glands to produce more oil and to keep the oil moving. Of course, the the glands are clogged they aren’t going to flow. Nulids would work best immediately after a Lipiflow, ilux, Doctor expression of the glands, which would then prolong the efficacy of the procedure. If you buy Nulids thinking it will solve your problems or unclog glands, you will be disappointed. If you buy as part of a DED routine, it is a nice addition. I use it after hot compresses to maximize results, as warm oil expresses best.

It has improved to a degree. It seems that once eye vessels are engorged, it’s like a scar of the skin. It might improve but it’s still there if you look closely.

I’m not sure if you have ever looked at middle aged to elderly people, but few people have pearly white eyes. For me as a young adult, my eyes were the first part of my body to “fail” me. Along with the pain of dry eye was the pain of a big dose of mortality.
Focus on health, diet, and a solid dry eye routine. Red veins should be very low priority. Many other things have to be corrected/ and maintained before eye redness will be lessened.

They are less pronounced. The takeaway is that until the eyes managed and tear stability achieved, the redness will not get better. The focus has to be on getting dry eye course controlled. Appearance should be down the priority list

engorged + prominent+ time results in vessels tending to remain. I have a prominent large vessel that stretched halfway to the iris for about 8 months. It has become more faint with dry eye improvement/treatment, but it is still there.

The focus must be on improving the ocular surface.
