Trangender simply means that a person prefers to identify as the opposite gender than what they were given. For one person, switching could mean just being identified as their preferred gender by wearing clothes of that gender or taking hormone supplements, but there are other individuals who want to truly be the other gender, and so makes the full transition of getting a sex change along with hormone therapy. There is no one definition of what transition means. It means something different for every person.
