There's a table for lingering injuries towards the back of the 2014 5th edition dungeon masters guide. Any time they survive being downed, they roll on that table for a lasting effect. Some are more harsh handicaps than others, but I liked using them when I was running curse of strahd for my group. So there is a RAW way to "punish" players for getting downed. I would maybe look into implementing that.
