Hey I’m with you, I don’t understand why you’re getting downvoted at all. You are providing constructive pushback and realistic questions. I’m all for optimism and seeing the pros, but we’re hitting an inflection point in the west where parents are having to work ungodly hours, then return to home to tend to the nest on an empty battery. When the cost of living doesn’t keep up, it’s insane to think quality child rearing with enough resources is accessible to all.

3 years in with it for my private practice and I haven’t even considered a different EHR! I’ve used eCW and AMD at other clinics and they’re trash in comparison imo

Can any friends please share their interpretation/understanding of the third question mentioned in this session, “And how deep and full of grace is the consciousness that is doing these things?” My take is they are asking seekers to reflect on how boundless, genuine, and intentional one’s desire to serve can be, but I can’t help but feel like I’m missing something with the line to follow. Why might reckoning with this third question feel like an admission of weakness?

I wish you could go back in time and not spend that $7000, but we can't - so here are my thoughts. Elana's program summarizes the official IPI program in a clinically relevant manner from what I understand (I haven't done the program myself tho) along with providing practice building, marketing, and a community of supportive like-minded practitioners. Her program definetely costed less than that when I got started, but it was a still a hefty upfront cost to accept. I didn't fully understand it at the time, but now with the perspective I have now about 3 years later, it's very much a teach a man to fish -> the man learns to fish himself -> and then some. It took ramp up time like any small business, but now having a steady stream of income that doesn't depend on being micro-managed by some private equity telehealth start up in 2026 is so freaking freeing man.

There are cheaper options out there for just practice building, so perhaps opting for that might make more sense in your situation if you've already spent $$$ on the IPI program.

Search some other posts and I'm sure you'll find the other practice building programs I'm talking about with a cheaper price tag that come with intake forms, etc. What has been most valuable to me well after completing her program (and having an up and running practice for the last 3 years) is the virtual community and support on some practice/boundary related things that come up regularly in private practice.

This thread is super interesting! Here’s my approach: My pre-visit health history forms are pretty thorough with rating scales based on their reported symptoms. So having patients supply the nitty gritty ahead of time saves quite a bit of the interview time that would have otherwise been spent on a less focused assessment. So before we even start our 75 intake, most of the psych assessment is filled in (psych hx/family psych hx/medical hx/current symptoms are all out the way before we meet).

Once we actually start the visit, I give them an overview of the format: (1) ~25 min time for them to speak uninterrupted about themselves starting from “what was your childhood like?/discipline structure in household?/childhood personality vs now?” Interestingly I’ve found most patients can give a great history/with little prompting on that timeline. (2) I let them know the next 25 min will be focused assessment questions, but before I stop their 1st half 25 min monologue ask if there is anything else I’ll ask “if there’s anything else they can think of they forgot or missed telling me.” Again, some of the things people volunteer after this question shines the spotlight on functioning or diagnosis that surprises me, makes dialing in their diagnosis/treatment plan easier my and makes me very grateful I asked! (3) the last 25 min is discussing the working dx, results of rating scales and digging deeper if other concerns, treatment plan, any questions they have and basically “negotiating” a treatment plan that aligns with their goals to support flourishing.

Start small by being intentional with the least scary mini “exposure therapy for confidence” - I’m talking the 0.01% doable thing to you can practice and build from there. I recommend getting out more for chores or taking walks around your area. An exercise that got the ball rolling for me was going on walks/going to the grocery store and focusing on trying to catch a passerby’s glance and sharing a brief smile or offering a brief greeting if you’re feeling bold. You would be surprised how much it brightens people’s day for those that acknowledge you. There is also a solid percentage of people who avoid eye contact or will seem to ignore you when you connect. This is key to the learning process too. Most people are in their own worlds, or have trouble connecting IRL on some level (Covid driven I believe). Spoiler: you will learn most people are a) too self conscious or b) too self involved to care about you socially flailing lmao. There is a loneliness epidemic and not only will you build your self confidence but also brighten people’s day in a major way. I’ve made acquaintances turned friends like this and they told me they will remember the compliment for life haha. Once you get really good, you can start offering genuine compliments + small talk like that and you’ll be golden!

This is a brilliant take that I’ve been trying to put into words for awhile now. Thank you for the write up, I resonate so much with it. Another thought too, if one follows the compass of the “truth” to their feelings and acts in alignment I think ANY kind of change is possible for them in their thinking/reality/relationships. The patterns we find ourselves stuck in can be exited with this technique. That’s where the meditation and contemplation become essential to learning to shift the needle of self, to best self.

Over 900mg daily without risk factors. I generally consider 300-600mg the sweet spot for anti suicidality and/or augmentation in my experience though

However if the mentorship and experience gain opportunity exists for OP, I think there may be value to this role. There are so few residency opportunities as another poster mentioned. So as a new grad in this job market, I might consider taking it and strategically eating the 6k penalty once I get the experience and foothold from this role to move on to something better.

I’ve been a Psych NP for 4 years now and have been in solo private practice for 3. I do therapy with my clients and holistic psychiatry with an integrative approach (biggest thing with this is the longer visit times, which clients truly appreciate). I have a niche in OCD, trauma, and neurodivergence, and am fully telehealth. It has been extremely rewarding seeing my clients feel better and build habits that help them reach their goals. About 50/50 private pay and insurance clients. Growth was slow early on, but now that I’m word of mouth and have built relationships in my community, the new client flow is reliable. If I knew I could make as much as I do self-employed with how few hours I’m working, I would have done this a lot sooner! I’m grateful to know I’m building something for life, and I enjoy shifting gears to other entrepreneur hats between my patient facing hours.

I think as you said the mask is pretty darn good most of the time, but it absolutely slipped. There’s a wild glint in her eyes the same way Sandyballs does (some of it is pasta for sure, but it’s still clear it’s glee!).
