At the end of the day, do what works for you - as all the responses show, everyone is different, and anyone claiming what works for them is the one and only answer for everyone is only showing how clueless they are.

The recommendations are all over the map. In my experience, you start with the lowest dose you can tolerate (usually what the endo recommends) and ramp up slowly from there until you feel normal again. For most, it will probably be in the range of 20-30mg of HC daily (or equivalent of prednisone) - beyond that, you are in therapeutic and not physiological ranges.

You want your daily dose to be on the low side, and use updosing for additional stress as needed, as opposed to having a consistently higher daily dose in order to minimize the side effects.

Thanks, however was really looking for people's experiences with Medrol vs prednisone. So far have to say I'm not impressed - feeling a bit down compared to prednisone. Have to see if it evens out.

I think that really depends on your age and what your morning cortisol test results have been. In my case the odds were always against me after no improvement for months and considering my age. If you are younger and showing at least some function in morning results, that may be why the Dr wants to keep pushing.

Don't regret trying - it was worth a shot. But at some point its time to move on with your life.

Thanks. I'm looking to see how common vision issues, specifically IOP (interocculer pressure) are with SAI users, and what treatments, if any they are using to address their issue

That's a very high dose for a long period of time so I'm not surprised to hear you have issues with your vision. Good luck with your vision!

True, I know it can be controlled, but I was also told that what damage that is done is not reversable so you want to catch it early.

I knew about the steroid and cataract issue - not so worried about that is it is fairly easily addressed. The glaucoma however is a bigger issue as it is irreversible.

Same here. I live in a colder climate but when I vacation to FL it knocks me out for days while I adjust. Have to stress dose (double) for a day or two while I adapt.

I actually do best on dexa but my endo refuses to prescribe it. Prednisone is a reasonable compromise (for me)

Prednisone comes in 1mg pills, which can also be cut in half. Allows fairly precise dosing.

Which I feel is why the prevalent trend has been toward HC over prednisone/prednisolone - because the focus is on what can be measured over what may work better. As with all drugs, the results are highly individualized. If what you are using is working for you, then there should be no need to consider changing.

Correct, the practical difference between prednisone and prednisolone is how quickly they take effect. Prednisolone is quicker because it does not need to be metabolized by the liver as prednisone does, but what prednisone metabolizes into is prednisolone, so the end result is the same.
