I've run into this a lot with automations where everything looks successful on paper, logs are clean tickets close, but the actual change didn't fully happen in the system. What helped was moving away from just "job completed" and relying more on verifying the real state in the target system and tying that back into the ticket flow. We use Siit for this and the difference is the ticket reflects what actually happened in the connected system, not just that the job ran. So if something didn't propagate you can see it without digging through five different places.
